护理-封闭式吸痰管操作规范
一、目的
1、使病人气道回路处于相对封闭状态。
2、避免医务人员因多次暴露气道口吸痰而增加肺部感染机会。
二、评估病人
了解病人病情及气道分泌物情况
三、准备
1、护士准备：着装整齐，洗净双手，戴口罩
2、病人准备：取适当体位，气切护理后《抽吸病人人工气道及口腔分泌物》
3、用物准备：封闭式吸痰管
4、环境准备：清洁、舒适
简要步骤：查对——解释——气管切开护理后更换封闭式吸痰管——连接氧气管或呼吸机螺纹管——吸痰后完全退出气管导管——签字——整理用物
五、用物整理：
将废弃的封闭式吸痰管放置于医用垃圾袋
六、护理指导：
指导病人勿随意调整管道的位置，如有不适，及时通知护理人员。

注：密闭式吸痰管与气切套管连接处有二个注液口，近端（靠近气切处）目前为白色、远端目前为蓝色。
注意事项：
1. 使用前检查中心吸引压力是否安全。
2. 插入真空表前，真空表模式开关应置于“OFF”档，逆时针调节旋钮转至零位。
3. 压力调节：成人300-400mmHg；小儿250-300mmHg。
4. 吸痰时间10-15s/次，间隔3-5min。
5. 使用一次性吸引袋则袋内吸出液体至容量的2/3应及时更换。
6. 治疗盘内用物应保持无菌，吸痰时遵守无菌操作原则。
7. 操作中注意观察患者面色、呼吸情况，及时监测脉氧，如有异常即刻停止吸痰，并
予相应处理。
8. 操作中注意与清醒患者的交流，切忌忽视患者的感觉及社会人角色。

2．在使用中心负压吸引过程中，要定期检查真空压力表是否在正常范围。
3．科室配备电动吸引器，定点放置。
4．定人定期（每周）检查、维护，做好维修和维护记录。
5．在使用过程中，如遇停电、真空压力不足、设备故障等突发情况时，应立即启用备用
电动吸引器或使用50ml注射器给患者吸痰。
以保持患者呼吸道通畅，对清醒患者做好心理护理。
6．使用过程中严密观察患者有无缺氧或其它生命体征变化，配合医生完成各项抢救措施。
并准确记录。
7．故障的吸引装置悬挂“仪器故障”牌，定点放置，及时送至仪器维修部门，做好交接班和维修记录。
