髋臼成形截骨术治疗大龄髋臼发育不良

【摘要】髋臼发育不良是先天性髋关节脱位的一种类型，因髋臼发育差造成髋臼
对股骨头的包容不够，股骨头向外移位，下肢负重力线改变，髋臼载荷面相对缩小，使下肢负重应力集中，造成髋关节局部关节软骨过早磨损，继续发展则成为骨关节炎，日后表现为跛行、髋痛及关节活动受限等症状。
髋臼成形截骨用于治疗髋臼发育不良，通过在髋臼外截骨后，旋转髋臼面向前外侧旋转，使之覆盖股骨头，增大了浅小的关节臼对股骨头的包容及载荷面，同时矫正股骨头的位置，

髋臼周围截骨术治疗髋臼发育不良李华;王云清;魏东【期刊名称】《临床骨科杂志》【年(卷),期】2011(014)006【摘要】目的探讨髋臼周围截骨术治疗髋臼发育不良的方法及疗效.方法对36例髋臼发育不良患者(43髋)行手术治疗,通过髋臼周围截骨、旋转髋臼向前外侧移位恢复髋臼的正确位置,并增加髋臼覆盖面.结果 36例均获随访,时间 6个月～3年.术后髋痛、跛行完全消失或有明显改善,髋关节活动范围基本正常.CE角和Sharp 角均基本恢复正常.结论髋臼周围截骨术可有效改善临床症状,恢复髋关节的生物力学特点,是治疗髋臼发育不良的有效方法.%Objective To discuss the method and effect of the treatment of acetabular dysplasia by periacetabular acetabular osteotomy. Methods Periacetabular osteotomy was performed to correct 36 patients( 43 hips )with dysplastic acetabulum through rotating the acetabulum anterolaterally, by which the position and acetabular coverage of the femoral head was restored to nearly normal. Results 36 patients with acetabular dysplasia were all followed up for six months to three years. After operation, in the majority, the hip limp or pain with exertion disappeared, and a satisfactory range of motion had been restored. CE angle and Sharp angle were restored to nearly normal value.
