5:菏泽试卷装订存档规范(试行)试卷是评价教学效果和反映教学质量的重要材料,为进一步做好试卷存档工作,特制定如下规范:一、试卷装订顺序及内容试卷装订顺序自上而下依次为试卷封面、考场记录、课程学生成绩单、空白试题、参考答案及评分标准、试卷分析、学生试卷。
具体内容要求如下:1、试卷封面试卷装订的封面由学校统一印制;封面的内容要填写规范,笔迹颜色要统一。
2、考场记录考场记录由学校统一印制;表内所有信息、签名要齐全规范。
3、课程学生成绩单课程学生成绩单统一由试卷分析系统打印,A4纸型,院长签字确认;课程成绩单应与网上录入上报的成绩单一致。
4、空白试题一份,纸型与试卷纸相同。
5、参考答案及评分标准一份,纸型与试卷纸相同。
6、试卷分析,试卷分析应包含考试成绩统计直方图,成绩结果分析,试题质量分析三个方面内容;纸型为70克A3纸。
7、学生试卷,学生试卷顺序为课程学生成绩单名单顺序(按学号小号在上大号在下),随班重修学生的试卷排在所随考试班级正常考试学生试卷后面,一并装订。
二、装订要求1、试卷装订必须整齐,阅卷前用考试密封纸沿密封线左侧装订;批阅后用试卷封面沿装订线左侧装订;装订要整齐、干净、美观。
2.试卷封面内容填写要完整,笔迹要工整、规范。
3、装订的学生答卷数与学生成绩单上的学生数要一致。
三、装订试卷的编号每个学期考试结束应立即对本学期试档进行整理编号卷存(各课程试卷编号见附表1),并填写《菏泽期末考试试卷存档一览表》(见附表2)。
四、艺术类专业试卷(作业)的装订与归档根据艺术类专业的特点,其试卷(作业)装订与归档规定如下:(一)各类理论课的试卷,严格执行学校试卷装订规范,并妥善保管。
(二)专业课试卷(作业)的装订与保管1、试卷(作业)分类管理(1)平面类试卷(作业)平面类试卷(作业)指试卷(作业)的完成稿以二维形态展示,如素描、平面构成、书法等。
这类试卷(作业)要拍成照片,保留电子档案。
如特别优秀可保留部分原作。

2、考场记录考场记录由学校统一印制;表内所有信息、签名要齐全规范。
3、课程学生成绩单课程学生成绩单统一由试卷分析系统打印,A4纸型,院长签字确认;课程成绩单应与网上录入上报的成绩单一致。
4、空白试题一份,纸型与试卷纸相同。
5、参考答案及评分标准一份,纸型与试卷纸相同。
6、试卷分析成绩录入结束后,从教务系统导出,并认真填写相关内容,做好对本门课程的反馈与总结。
7、学生试卷学生试卷顺序为课程学生成绩单名单顺序(按学号小号在上大号在下),随班重修学生的试卷排在所随考试班级正常考试学生试卷后面,一并装订。
二、装订要求1、试卷装订必须整齐,阅卷前用考试密封纸沿密封线左侧装订;批阅后用试卷封面沿装订线左侧装订;装订要整齐、干净、美观。
2.试卷封面内容填写要完整,笔迹要工整、规范。
3、装订的学生答卷数与学生成绩单上的学生数要一致。
三、装订试卷的编号每个学期考试结束应立即对本学期试档进行整理编号卷存(各课程试卷编号见附表1),并填写《菏泽期末考试试卷存档一览表》(见附表2)。
四、艺术类专业试卷(作业)的装订与归档根据艺术类专业的特点,其试卷(作业)装订与归档规定如下:(一)各类理论课的试卷,严格执行学校试卷装订规范,并妥善保管。
