【Abstract】 Objective To investigate the activation of nuclear factor鄄B (NF鄄B)and hypoxia鄄inducible factor鄄1 (HIF鄄1)in patients with obstructive sleep apnea鄄hypopnea syndrome (OSAHS). Methods Subjects were classified into mild鄄to鄄moderate OSHAS group (n = 16), severe OSAHS group (n = 14) and the matched control group (n = 30). Gene and protein expressions of NF鄄B and HIF鄄1 were measured by RT鄄PCR, Western blot in peripheral blood mononuclear cells (PBMC). Results NF鄄κB p65 mRNA and NF鄄κB p65 protein,HIF鄄1α mRNA and HIF鄄1α protein in PBMC were significantly higher in severe OSAHS group than those in mild – to鄄moderate group and control group (P < 0.01). But there were no significant difference in NF鄄κB p65 mRNA and NF鄄κB p65 protein between mild鄄to鄄moderate group and control group (P = 0.068, P = 0.254 respectively). Only gene (P < 0.05) not the protein (P = 0.777) of HIF鄄1α was higher in mild鄄to鄄moderate as compared with control group. Both NF鄄κB p65 mRNA and HIF鄄1α mRNA were positively correlated with AHI (r = 0.493, P = 0.006, r = 0.508, P = 0.004), while negatively correlated with nighttime lowest blood oxygen saturation (LSaO2)(r = -0.488, P = 0.006, r = -0.46, P = 0.011). There was a positive correlation between NF鄄κB p65 protein level and AHI (r = 0.669, P < 0.001). HIF鄄1α protein level was positively correlated with AHI, ODI (r = 0.628, P = 0.001; r = 0.480, P = 0.018). There were positive correlations between NF鄄κBp65mRNA and HIF鄄1αmRNA (r = 0.543, P = 0.002), NF鄄κBp65 and HIF鄄 1α protein respectively (r = 0.716, P < 0.001). Conclusions As the gene and protein of NF鄄κB and HIF鄄1 were up鄄regulated in patients with OSAHS, and also positively correlated with the severity of sickness. We conclude that both NF鄄κB and HIF鄄1 were involed in the pathogenesis of OSAHS.

摘要 目的:探讨核因子-κB(NF鄄κB)与缺氧诱导因子-1(HIF鄄1)在阻塞性睡眠呼吸暂停低通气综合征 (OSAHS)的激活情况。 方法:研究对象分为轻中度 OSAHS 组(16 例)、重度 OSAHS 组(14 例)及对照组(30 例), 通过实时荧光定量 PCR 法(Real鄄time)及 Western blotting 法分别测定外周血单个核细胞(PBMC)的 NF鄄κB、 HIF鄄1mRNA 水平和 NF鄄κB、HIF鄄1 蛋白水平。 结果: 重度 OSAHS 组 NF鄄κBp65mRNA 及 NF鄄κBp65 蛋白表达、 HIF鄄1αmRNA 及 HIF鄄1α 蛋白表达均显著高于对照组及轻中度 OSAHS 组 (均 P < 0.01);NF鄄κBp65 mRNA 及 NF鄄κBp65 蛋白表达轻中度 OSAHS 组与对照组无明显差异 (P 值分别为 0.068 及 0.254), 轻中度 OSAHS 组 HIF鄄1αmRNA 明显高于对照组(P < 0.05)但 HIF鄄1α 蛋白表达无明显差异(P = 0.777)。 NF鄄κBp65mRNA 及 HIF鄄1αmRNA 的表达与呼吸暂停低通气指数(AHI)呈正相关(r = 0.493, P = 0.006, r = 0.508, P = 0.004),与 夜间最低血氧饱和度(LSaO2)呈负相关(r = -0.488, P = 0.006, r = -0.46, P = 0.011)。 NF鄄κBp65 蛋白表达与 AHI 呈正相关(r = 0.669, P < 0.001)。HIF鄄1α 蛋白表达与 AHI、氧减指数(ODI)呈正相关(r = 0.645, P = 0.001; r = 0.480,P = 0.018)。 NF鄄κBp65mRNA 与 HIF鄄1αmRNA 呈正相关(r = 0.543,P = 0.002),NF鄄κBp65、HIF鄄1α 蛋 白表达呈正相关(r = 0.716,P < 0.001)。结论:OSAHS 患者 NF鄄κB 及 HIF鄄1 的基因和蛋白表达均出现上调,且 患者病情越重上调越明显,表明 OSAHS 的发病与两者均有关。

学者认为该炎症通路主要由 NF鄄κB 调控; 也有学 者发现 HIF鄄1 依赖的适应性通路也可被间歇低氧 诱导;两条通路是否参与发病及两条通路的相互关 系等问题尚未完全清楚。本实验通过测定外周血单 个核细胞(PBMC)两转录因子在 OSAHS 患者中的 表达, 探讨 OSAHS 患者缺氧通路的激活情况及发 病机制。 1 对象与方法 1.1 研究对象 2010 年 10 月 至 2011 年 8 月 在 广 东 省 人 民 医 院 行 ALICE 4 多 导 睡 眠 图 仪 (Respironics, 美 国 ) 检 查 者 , 分 为 轻 中 度 OSAHS 组、重度 OSAHS 组及对照组。 OSAHS 组按中华医 学会呼吸病学分会睡眠呼吸疾病学组制定的 OSAHS 诊断标准[3],其中轻中度组 16 例,重度组 14 例。 对照组为无 OSAHS 及 COPD 者,共 30例。 其 中轻中度组 11 例,重度组 13 例,对照组 14 例,进 行了蛋白分析。
