CM易发生全身转移，最常亦最早累及肝脏(89%)，有研究显示即使眼部原发病灶得到了早期控制，其5年和10年转移率仍高达25%和34%[2]。

一旦CM发生转移，由于尚未有针对转移性CM的有效治疗手段，约80%的患者会在1年内死亡[2-3]。

有统计显示CM的初诊漏诊率高达23%，而漏诊及误诊将导致治疗的延误，约10.9%的患者治疗延迟大于6个月[4]。

因此CM的早期诊断与干预显得尤为重要。

1 病例资料患者，男，40岁，因“右眼视物不清1月”在外院住院治疗，诊断“右眼视网膜脱离，右眼脉络膜脱离，右眼占位性病变”，于局麻下行“右眼玻璃体切除+气液交换+硅油填充术”，术中于上方灰白色隆起处(角巩缘后6 mm)行巩膜穿刺见血性液体。

术后第1天患者自觉右眼视物不清较术前加重，行眼眶+头颅MR平扫提示右侧玻璃体内2个短T1长T2异常信号，考虑为出血，予以抗炎、止血等对症治疗。

术后第7天，患者至我院门诊就诊，专科检查：右眼视力指数/10 cm，眼压18 mmHg，角膜透明，前房深度可，PACD>1CT，闪辉(±)，瞳孔圆，直径4 mm，对光反射存在，晶状体轻度混浊，硅油下眼底颞上方网膜下/脉络膜下明显实性隆起，诊断“右眼球内占位性病变性质待查，玻切+硅油填充术后”，建议患者行PET检查排除眼内肿瘤性病变，患者未采纳，遂予以抗炎、活血化瘀等对症治疗，半月后复诊时见实性隆起似较前稍低平，故继续同前治疗，后复诊2次，实性隆起未见明显变化，再次建议完善PET检查，患者仍未采纳。

本文报道1例CMM的临床资料及超声诊断并结合文献复习，旨在探讨CMM的超声声像图及彩色多普勒表现，为其诊断及治疗提供更多帮助。

《新英格兰医学杂志》2015年5月31日在线先发Nivolumab联合伊匹单抗或单药治疗初治黑色素瘤的疗效对比背景Nivolumab（一个程序性死亡1[PD-1]检查点抑制剂）和伊匹单抗（细胞毒性T淋巴细胞相关抗原4[CTLA-4]的检查点抑制剂）已显示出在转移性黑色素瘤的互补（治疗）活性。

在这项随机、双盲、3期临床研究中，比较nivolumab单药或nivolumab加伊匹单抗与伊匹单抗单药在转移性黑色素瘤患者中的疗效。

方法我们将945例初治、不可切除的III期或IV期黑色素瘤以1：1：1的比例进行分配，分别接受nivolumab单药、nivolumab加伊匹单抗或伊匹单抗单药。

主要终点是无进展生存期和总生存期。

关于无进展生存期的结果在此呈现。

结果nivolumab加伊匹单抗组的中位无进展生存期为11.5个月（95％置信区间[CI]，8.9〜16.7），伊匹单抗组单药为2.9个月（95％CI，2.8〜3.4）（死亡或疾病进展风险比，0.42; 99.5％CI，0.31〜0.57; P <0.001），nivolumab单药组为6.9个月（95％CI，4.3〜9.5）（与伊匹单抗比较的危险比，0.57; 99.5％CI，0.43〜0.76; P <0.001）。

在PD-1配体（PD-L1）阳性的肿瘤患者中，nivolumab加伊匹单抗组和nivolumab单药组的中位无进展生存期为14.0个月，但在PD-L1阴性的肿瘤患者中，联合治疗组比nivolumab单药组的无进展生存期更长（11.2个月[95％CI，8.0〜未到]对比5.3个月[95％CI，2.8〜7.1]）。

3级或4级治疗相关性不良事件在nivolumab单药组的16.3％患者中发生，在nivolumab加伊匹单抗组的55.0％患者中发生，在伊匹单抗单药组的27.3％患者中发生。

结论在初治、转移性黑色素瘤患者中，nivolumab单药或与伊匹单抗联合治疗比伊匹单抗单药有显著更长的无进展生存期。
