早期无创正压通气在创伤性湿肺患者中的应用（作者:___________单位: ___________邮编: ___________）【摘要】目的：探讨早期无创正压通气在创伤性湿肺患者中的疗效。

方法:采用双水平气道内正压（BiPAP）无创通气治疗早期创伤性湿肺患者，动态观察患者动脉血气分析和呼吸频率,并与治疗前相比较。

结果:采用BiPAP无创通气治疗后，较前面罩吸氧，pH 值和PaCO2无明显变化（P0.05)，PaO2和呼吸频率明显改善（P0.05），未见明显不良反应。

结论：早期BiPAP无创通气能有效改善创伤性湿肺患者的氧合，是早期治疗创伤性湿肺的有效方法，但是一旦病情进展，应及时采取有创机械通气。

【关键词】创伤性湿肺;早期无创正压通气;双水平气道正压［ABSTRACT］Objective: To explore the effect of non invasive ventilation at early stage on traumatic wet lung.Methods: Non invasive ventilation with bi level positive airway pressure (BiPAP) was carried out to treat patients with traumatic wet lung. With arterial blood gas analysis and breathing frequencymeasurement, all patients were under dynamic monitoring, and the therapeutical comparison was carried out.Results: After treatment, there was no significant change in pH and PaCO2 comparing with those of patients with oxygen mask (P0.05), but remaked improvement in PaO2 and breathing frequency (P0.05). Besides, no side effect occurred.Conclusions: Non invasive ventilation at early stage can effectively improve oxygenation status in patients with traumatic wet lung, and is effective treatment. （3）降低呼吸功耗，缓解呼吸肌疲劳：由于气道阻力增加、呼吸系统顺应性降低和内源性呼气末正压（PEEPi）的出现，呼吸功耗显著增加，严重者出现呼吸肌疲劳。

呼吸机分类睡眠呼吸机：通气模式为CPAP，也称为睡眠无创呼吸机无创睡眠呼吸机CPAP呼吸机正压呼吸机持续正压呼吸机单水平呼吸机单水平持续正压呼吸机。

一般来说，单水平手动呼吸机以其简单易行、治疗效果好，已成为治疗睡眠呼吸暂停低通气综合征最有效最广泛的方法。

单水平自动呼吸机的特点是可随患者气道阻力的变化相应地自动升高和降低输出压力。

这种机型特别适用于睡眠呼吸暂停轻、中度的患者，而且市面上的单水平自动呼吸机都带有CPAP模式。

双水平呼吸机的特点是有吸气压和呼气压两个压力。

它更适用于中枢性睡眠呼吸暂停低通气综合征、不能耐受CPAP治疗的阻塞性呼吸暂停低通气患者、以及伴随其他肺部和心脏引起的呼吸障碍的患者。

方法 选取2022年1—12月江苏省沭阳仁慈医院收治的60例慢阻肺急性加重期伴呼吸衰竭患者为研究对象，依据随机数字表法分为对照组和观察组，各30例。

对照组采用BIPAP 治疗，观察组采用硫酸特布他林+BIPAP 治疗，比较两组患者的免疫功能水平、血气指标、肺功能指标。

结果 观察组患者的各项免疫功能水平和血气指标水平优于对照组，差异有统计学意义（P 均<0.05）。

观察组患者的肺功能指标高于对照组，差异有统计学意义（P <0.05）。

结论 慢阻肺急性加重期伴呼吸衰竭疾病病情重，危及患者机体健康，采用BIPAP 与硫酸特布他林联用，可快速恢复免疫功能，改善血气，并提高肺功能。

They were randomly divided into a control group and an observation group, with 30 cases in each group, using a random number table method. The control group received BIPAP treatment, while the observation group received ter⁃butaline sulfate + BIPAP treatment. The immune function levels, blood gas indexes, and lung function indexes of the two groups of patients were compared. Results The levels of various immune functions and blood gas indexes in the ob⁃servation group were better than those in the control group, and the differences were statistically significant (all P <0.05). The lung function indexes of the observation group was higher than those of the control group, and the differ⁃ence was statistically significant (P <0.05). Conclusion The acute exacerbation of COPD accompanied by respiratory failure is serious and endangers the health of patients.
