经尿道前列腺电切术后尿道狭窄的防治研究作者：杨兴业覃光刘伟来源：《中国实用医药》2013年第28期【摘要】目的探讨经尿道前列腺电切（ TURP）术后尿道狭窄的防治。
方法回顾TURP 治疗良性前列腺增生（BPH）患者1120例，对术后出现52例尿道狭窄进行总结。
结果 1120例患者，术后1月～2年发生尿道狭窄52例（4.6 %）。
前尿道狭窄 20例（38.4%），后尿道狭窄 16例（30.8%），膀胱颈挛缩16例（30.8%）。
52例随访1月～2年，平均12个月。
单纯尿道扩张术28例， 19例（67.9%）完全治愈，9例改行尿道内切开术；尿道内切开加尿道扩张术13例完全治愈，膀胱颈部挛缩者11例，行膀胱颈部挛缩电切术均治愈。
结论尿道狭窄是TURP术后常见并发症，术后随访，早期治疗是关键。
【关键词】良性前列腺增生；经尿道电切术；尿道狭窄；防止岑溪市人民医院自2009年7月～2012年12月经尿道前列腺电切术治疗良性前列腺增生患者1120例，术后出现尿道狭窄52例。
现将尿道狭窄的防治经验报告如下。
1 资料与方法1. 1 一般资料本组52例，年龄53～93岁，平均（70.5±8.5）岁。
患者手术后5～7 d，拔出尿管，小便顺畅，出院后1～3月，患者出现尿流变细或分叉，尿不净，严重者出现小便困难，尿潴留等。
经尿道造影或膀胱尿道镜检查证实尿道狭窄。
狭窄段长0.3～3.0 cm，平均1.5 cm。
其中20例患者为前尿道狭窄， 16例为后尿道狭窄， 16例患者为膀胱颈挛缩。
1. 2 治疗方法1. 2. 1 单纯尿道扩张术患者行造影时若狭窄部位在2 cn范围内，且尿道镜检查发现仍有细腔可行此手术。
一般扩张术6～8次， 1次/周。
若仍失败，可改行尿道内切开+尿道扩张术。
1. 2. 2 尿道内切开术狭窄段1. 2. 3 膀胱颈部成形术膀胱颈或前列腺尿道部位的狭窄，用电刀在3点或12点切开尿道，插入膀胱，彻底切除瘢痕组织。
1.简述尿路梗阻的治疗原则。
尿路梗阻的病因很多，治疗方法也各异。
但总的治疗原则是解除梗阻引流尿液。
1、若肾功能正常，最好是解除梗阻和治疗同时进行。
如前列腺增生、肾盂输尿管连接部狭窄等，可一期解除梗阻、去除病因。
2、如果病因和梗阻不能同时处理，可先行尿流改道解除梗阻，病情允许后，再行病因治疗。
3、如肾功能有严重损害，应立即解除梗阻，治疗合并症，最限度恢复肾功能。
4、急性梗阻，如手术中误扎输尿管，结石阻塞等，应早做诊治，以保护肾功能。
5、如肾脏严重积水，静脉尿路造影或肾图扫描证明已无功能，或合并难以控制的感染，行肾切除。
2.简述乳糜尿的手术治疗方法。
以往对乳糜尿的手术治疗采取肾切除术、肾被膜剥脱术等，因破坏大，疗效差，现已废弃不用。
目前常用的手术方式有：1.肾蒂淋巴管结扎术手术游离肾蒂淋巴管、脂肪、结缔组织，切断，两端分别结扎。
2、胸导管与半奇静脉吻合术手术操作较难，而且效果不可靠。
3、大隐静脉分支与腹股沟淋巴结吻合术手术损伤小，暴露表浅，吻合通畅后，因短路分流减轻了负担，乳糜尿得以停止或减轻。
4、精索淋巴管与静脉吻合术这也是一种淋巴静脉内引流术，使扩张高压的淋巴管内的乳糜液经精索淋巴管流入精索静脉内，减轻腹膜后淋巴液淤积，使淋巴瘘孔得以闭合，临床效果满意。
3.简述肾癌的诊断要点。
出现血尿、疼痛、肿物三联征时，肾癌不难诊断。
许多病人虽有转移但尚无明确的症状。
2、B超扫描：可发现无任何病象的早期肾癌。
3、X线检查包括平片、排泄性或逆行泌尿系造影。
可见到肾轮廓增大、肾区钙化、肾盂肾盏变形或不显影。
CT检查：可发现未引起肾盂肾盏改变的隐性肾癌。
4.CT可准确诊断肾静脉侵犯、下腔静脉内癌栓肾周围扩散、淋巴结转移和附近脏器受累等。
5、肾动脉造影检查常出现异常血管征象，可发现泌尿系造影时肾盂肾盏无变形的肿瘤。
6、下腔静脉造影检查可明确有无癌栓以及癌栓的大小和范围，有利于手术摘除。
4.尿道扩张术可发生那些并发症？怎样预防出血多发生在狭窄的膜部，若暴力强行通过，则可造成黏膜挫伤或破裂出血。

1例男性前尿道狭窄在输尿管镜下行尿道扩张术的手术护理目的探讨经尿道输尿管镜下利用筋膜扩张器行前尿道狭窄扩张术的手术护理技术技巧及手术中的配合要点。
方法分析1例男性前尿道狭窄经输尿管镜下行尿道扩张术的手术护理技术技巧及手术中的配合要点。
结果该例患者手术成功，手术时间1.5h，术中出血量约10ml，术中出血少，创作小，可在直视下行尿道扩张术，手术安全，减少手术并发症。
未发生因手术器械物品准备不齐全而导致改变手术方式。
患者尿道极度狭窄首次扩张成功，免行尿道狭窄粘连段切除后尿道吻合术。
患者住院18d，留置三腔导尿管10～14d。
结论经尿道输尿管镜下尿道扩张术的手术护理配合对保证手术成功有重要意义。
标签：前尿道狭窄；经尿道；输尿管镜；扩张术尿道狭窄是泌尿系统常见病，多见于男性，临床上常见有先天性尿道狭窄如先天性尿道外口狭窄、尿道瓣膜、精阜肥大、尿道管腔先天狭窄等，炎症性尿道狭窄常因尿道管腔感染而引起粘连狭窄。
外伤性尿道狭窄多因损伤初期处理不当或处理不及时所致。
该例患者为男性，有骑跨伤史，排尿困难5个月需行膀胱造瘘术引流尿液。
为了手术的安全和保证手术成功，对手术中的手术护理配合总结分析，取得良好成果，现介绍如下：1资料与方法1.1一般资料患者，男性，41岁，农民工，于2013年03月因在工程施工中不慎跌落，导致尿道骑跨伤，无合并骨盆骨折和其它部位的骨折损伤，排尿困难进行性加重，出现尿潴留，膀胱出现多个憩室，双侧输尿管扩张，需行膀胱造瘘术以引流尿液。
1.2麻醉方式腰硬联合麻醉。
1.3手术方法患者行膀胱截石位，无菌操作及显视器的直视下经尿道轻柔置入输尿管镜，见到狭窄段尿道后置入斑马导丝到达膀胱，使用筋膜扩张器及金属扩张器逐步扩张狭窄段尿道至扩张到F9金属尿道扩张器的宽度。
直视下检查膀胱及尿道无明显出血点后留置三腔导尿管，术毕安返病房。
1.4结果患者手术取得成功，创作小，可在直视下行尿道狭窄段的扩张，手术安全，患者免于行尿道狭窄段的切除术。

方法对18例TURP术后排尿困难的病人分别采用尿道扩张、Otis刀切开、直视下尿道内切开、重新TURP、膀胱颈切开、腔内碎石、间歇性清洁自家导尿及药物治疗。
结果所有病人排尿困难的症状均明显改善，无尿失禁等严重并发症。
结论排尿困难是TURP术后的常见并发症，可采取防范措施，微创处理安全有效。

1 资料与方法1.1 一般资料本组18例，均为因症状性BPH施行了TURP的病人，年龄54～90岁，平均73.2岁，术后出现排尿困难的为拔管后当日～3年，平均4.2个月。
其中发生尿潴留6例，其余12例均有明显的排尿困难，最大尿流率为2.1～11.7ml/s，平均为7.2±2.6ml/s，残余尿量46～383ml，平均242±79ml。
经B超、尿道探试或造影、膀胱尿道镜及尿流动力学检查诊断前尿道狭窄7例，后尿道狭窄2例，膀胱颈挛缩3例，组织残留过多2例，神经源性膀胱2例，尿道狭窄合并结石1例，组织条或血块阻塞1例。
1.2 治疗方法对所有患者均先行尿道扩张；尿道狭窄扩张无效者采用Otis刀12点位前尿道切开和（或）直视下尿道内切开；对膀胱颈挛缩者应用经尿道膀胱颈倒“Y”型切开加瘢痕组织切除；对组织残留过多的病例施行重新电切；对尿道狭窄合并结石先处理尿道狭窄，然后将结石推入膀胱再施行腔内碎石取石；对组织条或血块阻塞引起的排尿不畅给予膀胱镜下异物清除；对神经源性膀胱患者应用α受体阻滞剂和平滑肌收缩增强剂，残余尿量过多者采用间歇性清洁自家导尿。
