一例待行二尖瓣置换术患者的循证治疗

病例背景：
患者女性，45岁。
主诉：呼吸困难1月，加重1周，痰中带血2天。
现病史：1月前，患者做家务后出现呼吸困难，休息后可缓解，无喘息喘鸣、咳嗽咯血、胸闷胸痛，当时未予处理，此后上述症状持续并加重，1周前咳嗽较频繁，为呛咳，单声，咳白色粘液痰，快步行走及上楼梯时出现呼吸困难，无夜间阵发性呼吸困难，2天前无诱因下咳出粉红色痰，无发热、无恶心、呕吐，无腹痛腹胀，无肛周疼痛。于当地医院就诊，查胸片示“肺水肿”，患者为求进一步诊治，于今日到我院急诊就诊，查心脏彩超示“二尖瓣狭窄并关闭不全”，收入我科。患者自起病以来，大小便正常，饮食可，精神可，睡眠可，体重无明显减轻。
体格检查：体温：37.2℃；脉搏：95次/分；呼吸：22次/分；血压：128/72mmHg。一般情况尚好，自动体位。皮肤巩膜无明显黄染，肺部听诊双下肺广泛湿罗音，心脏听诊心尖处闻及收缩期吹风样杂音舒张期隆隆样杂音，向左腋下左肩胛处传导。腹部平坦，腹无压痛、反跳痛（-），肝脾肋下未触及，未及腹部肿块。双侧下肢无水肿，神经系统体征（—）。肛门及外生殖器未查。
入院后检查：心脏多普勒超声示：LAS:42mm LVD:42mm RVD:20mm RAS:29mm IVSD:10mm LVPWD:10mm AO:29 PA:25 TV(返流):3.0m/s MV(返流):3.5m/s EF:70%
B: 左房增大，余房室比例正常，二尖瓣瓣环增厚回声增强，，二尖瓣三尖瓣关闭裂隙。肺动脉内径增宽。
CDFI: 收缩期二、三尖瓣瓣上呈以蓝色为主五彩镶嵌的返流束。根据连续多普勒三尖瓣返流压差估测肺动脉压约46mmHg。
一、提出临床问题
患者先内科治疗缓解肺水肿，后需经外科治疗根治，现认为该患者需行二尖瓣置换术，在不考虑经济条件情况下，可以选择机械瓣膜或生物瓣膜。两者差异是：机械瓣膜使用时间长，而且不易劳损，短期再次手术机会小，20年后可能需要更换，但需终身服用华法林；生物瓣膜术后不需抗凝，视个体差异可能终身无需再次手术，或者因排斥会在短期内出现钙化需要再次手术。
