船员工作表现报告
CREW PERFORMANCE REPORT
Master, C/E and C/O should fulfilled this form in double copies by the end of Mar respectively. and Sep. One copy sent to crew department, the ot her kept onboard in master’s file No.17(B), C/O’s file No.25(D),C/E’s file No.(D) respectively
2、船长负责轮机长、驾驶员的工作表现评定；轮机长负责轮机部船员的工作表现评定；大副负责甲板部普通船员的工作表现评定。
Master makes the performance appraisal of the C/E and all Navigation Officers. C/E makes the performance appraisal of the engine department crew. C/O makes the performance appraisal of the deck hands (including Ch. Cook and Ch. Steward).
3、需要时，船长也可以对轮机长和驾驶员以外的船员工作表现进行评定。
If needs, master can make a special performance appraisal of any other crewmember except from C/E & Navigation officers.
4、评语分级如下(Grading of appraisal as follows):
VG= 非常好Very Good * G= 好Good F= 一般Fair P= 差Poor *
