原发性脊柱恶性淋巴瘤的影像学表现分析目的观察分析原发性脊柱恶性淋巴瘤的影像学表现,分析X线平片、CT、MRI的诊断价值。
方法选取我院2010年6月~2015年6月收治的7例原发性脊柱恶性淋巴瘤患者,均经穿刺或手术病理确诊,均接受X线平片、CT、MRI 检查。
结果本组7例患者均为非霍奇金淋巴瘤,累及18个椎体,其中胸椎4例,腰椎1例,胸腰椎1例,颈椎1例;3例伴有附件受累,5例软组织肿块,3例椎管内、硬膜外肿块;X线平片影像学表现为椎体骨质破坏,椎体压缩性骨质,无椎弓根影;CT平扫肿瘤为中等密度,增强后强化均匀,无坏死;MRI T1WI 肿瘤为低等信号,T2WI为低、等、高信号,增强扫描后中度强化。
结论(1)原发性脊柱恶性淋巴瘤以非霍奇金淋巴瘤较为常见;(2)肿瘤侵犯相邻椎体,软组织肿块较大且极少坏死,增强扫描肿瘤为中度强化;(3)X线平片诊断作用有限,MRI显示骨内病灶敏感性高于CT。
[Abstract] Objective To analyze the image manifestations of primary spinal malignant lymphoma and the diagnostic value of X-ray plain film,CT and MRI. Methods 7 patients with primary spinal malignant lymphoma who had received treatment in our hospital from June 2010 to June 2015 were selected as the research objects. All of the patients were confirmed by puncture or surgical pathology for diagnosis,which were given X-ray plain film,CT and MRI for examination. Results The 7 cases were all diagnosed with non-Hodgkin’s lymphoma,involving 18 centrum,which included 4 cases thoracic vertebra,1 case lumbar vertebra,1 case thoracolumbar vertebra and 1 case cervical vertebra;3 cases with affected adnexa,5 cases with soft tissue mass,3 cases intraspinal or epidural masses.The imaging of X-ray plain film was manifested with destruction of vertebral bodies,compression osteoporosis,without vertebral pedicle artifact;the tumor was detected by CT plain scan to be of moderate density,homogenous post-enhancement,without necrosis;MRI and T1WI reported low intensity of tumor,yet the T2WI reported both low and high density,and the post-enhancement was moderate. Conclusion(1)Non-Hodgkin’s lymphoma is one of the common primary spinal malignant lymphoma;(2)Tumors will involve adjacent centrum,produce larger soft tissue mass and less necrosis,and the post-enhancement result is moderate;(3)X-ray plain film is with limited roles and the sensitivity to bone lesion of MRI is higher than that of CT.[Key words] Primary spinal malignant lymphoma;X-Ray plain film;CT;MRI;Image manifestations骨恶性淋巴瘤是少见的恶性肿瘤,是淋巴瘤晚期全身散播的表现,区分难度较大,骨恶性淋巴瘤包括原发性和继发性[1,2],原发性脊柱淋巴癌症状不明显,多为单一骨侵犯,预后良好;继发性脊柱淋巴癌指骨外淋巴组织恶性淋巴瘤侵入骨骼,病情严重,预后较差。
淋巴瘤影像鉴别1. MRI对脑转移瘤和多发性恶性胶质瘤的鉴别诊断脑转移瘤和恶性胶质瘤MRl的鉴别诊断要点有:①转移瘤患者年龄比胶质瘤大,常有原发瘤病史;②转移瘤易多发且位置表浅,瘤脑界面一般比胶质瘤清晰;③转移瘤单发时一,整个肿瘤均匀强化比胶质瘤多见,环状强化时转移瘤环壁相对规则;④胶质瘤多发时病灶相对集中。
但这些征象缺乏特异性。
脑实质内转移瘤的临床及MRI的表现具有以下特点:脑转移瘤多见于中、老年人,肺癌是其最常见的原发肿瘤,其他原发肿瘤还包括消化道癌肿、乳腺癌等,脑转移瘤可发生于脑实质、脑膜及脑室,内其中以脑实质最为常见。
脑实质转移瘤常位于皮髓质交界区,可多发也可单发。
病灶一般体积较小,无包膜,血液供应丰富,易于发生坏死、囊变及出血。
临床表现常随转移瘤所处部位不同而出现不同的临床表现,早期可无症状,随之可出现头痛、头晕、肢体无力、感觉异常以及颅内高压表现,病情呈进行性加重汇2一4J。
MR嚷现为病灶多发,大脑半球或小脑内均可发生,病灶位于皮髓质交界区或皮质内,呈膨胀性生长,多呈圆形及类圆形,其外缘较光滑、清晰,呈圆形或类圆形,大小一般为2. O一3.ocm,瘤内呈不均匀略长、长或等Tl及长、略长或等TZ信号,均有占位效应及水肿,常有坏死、囊变或出血。
当早期转移,病灶为单发时,肿瘤体积较小。
增强后病灶呈不均匀性明显或中等度强化。
胶质瘤在成人和儿童均可发生,成人以幕上多见,儿童位于幕下较多,但以20一40岁年龄组最常见。
胶质瘤多单发,但多发病例也不少见。
胶质瘤病灶多位于髓质或皮质下区,位置相对较深。
病灶大小不一,一般发现时较大,肿瘤周围水肿相对较轻l5]。
因为星形细胞瘤无包膜,一般呈浸润性生长,致使外壁毛糙不光整,与周围组织分界不清。
瘤体中心易发生囊变坏死,从而实质部呈一环状,不规则,环壁厚薄不均,内壁凹凸不平。
2.多发性硬化的MRI特征MS的临床诊断要求具有不同部位神经损害反复发作的特征,即俗称的“2+2”(2个部位, 2次发作),这势必需要包括较长时间的病程随访。
脊柱恶性血管源性肿瘤的影像表现及鉴别诊断张立华;袁慧书【摘要】Objective:To explore the CT and MRI appearances of malignant vascular tumor of spine.Methods:CT and MRI findings of 9 cases with malignant vascular tumor of spine confirmed by operation and pathology were retrospectively analyzed and summarized.Results:There were two cases of angiosarcoma and seven cases of epithelioid hemangioendothelio-ma (EHE).The tumors of 5 cases located in cervical spine,3 cases in thoracic vertebra,and 1 case in lumbar spine;the tumors were local in 4 cases,multicentric in 2 cases,and involved the adjacent vertebral body in 3 cases.The main CT fin-dings:osteolytic bone destruction with incomplete bone cortex could be found in allcases,residual bone trabecula within the lesions could be seen in five cases.MRI findings:all tumors were hypo-intense on T1 WI;On T2 WI,3 cases showed heteroge-neous signal,2 cases iso-intense signal and 4 cases hyper-intense signal;four tumors showed obvious enhancement and one showed moderate enhancement on post-contrastimages.Conclusion:Malignant vascular tumors of spine have some CT and MRI characteristics,including that the main location is cervical and thoracic spine,osteolytic bone destruction may be local or multicentric,some of them may have the characters of hemangioma and lead to spinal compression.%目的:探讨脊柱恶性血管源性肿瘤的影像表现。
脊柱淋巴瘤的CT及MRI表现脊柱淋巴瘤的发生,多为其他部位的淋巴瘤扩散导致,只有极少部分患者是原发性脊柱淋巴瘤。
目前在临床治疗脊柱淋巴瘤时,其首要前提是要及早诊断,确保临床诊断效果。
而大量研究资料显示,脊柱淋巴瘤临床误诊率高,而影像学准确诊断对脊柱淋巴瘤治疗方案及预后评估具有极其重要的作用。
因此对脊柱淋巴瘤,需选择更为有效的诊断方法提高患者诊断率。
本文以15例脊柱淋巴瘤患者为例,采取CT MRI诊断,其结果分析如下。
1 资料与方法1. 1 一般资料选取2012年5月〜2015年8月本院收治的15例脊柱淋巴瘤患者在经手术病理或穿刺等证实为脊柱非霍奇淋巴瘤[1] ;未合并脊柱其他恶性病变者。
其中男10例,女5 例;年龄16〜60 岁,平均年龄( 37.1 ±8.1 )岁;患者表现为病灶相应部位疼痛、感觉障碍等;部分患者伴腰痛、腿疼、肩部疼痛及胸部束带感;6例患者存在脊髓压迫症状;8例患者为原发性脊柱淋巴瘤,7 例患者为颈部、腹股沟、锁骨上区、腹腔、腹膜后淋巴结肿大病变导致;患者均自愿参加此次研究;诊断前未接受任何治疗。
1. 2 检查方法CT 检查:取Brilliance 16 排或GE 64 排CT扫描,患者取俯卧位,扫描参数:电压120 kV ,电流250 mAs 层厚5 mm 重建层厚1 mm 扫描时间5〜7 s , 视野(FOV 360 360 mm 矩阵512X512, 对患者采取轴位、矢状位及冠状位重建。
MRI检查:采取Sieme ns Son ata 1.5 T 磁共振机对患者予以扫描,行横断面、矢状面扫描。
扫描序列:快速自旋回波TSE T1WI 序列(TR600 ms TE 11ms ;快速自旋回波TSE T2WI序列( TR 2300 ms,TE 98 ms )。
对脂肪抑制序列颈胸椎使用反转恢复法,以脂肪饱和法对腰骶椎予以扫描。
扫描参数:层厚为颈椎、胸椎矢状位3 mm,腰椎、骶椎矢状位 4 mm,轴位 4 mm。
