工程项目承包商安全管理处罚细则

工程项目承包商安全管理处罚细则
XX项目承包商应遵照法律法规、XX公司要求和合同规定落实项目安全管理。对项目期间其员工及其分包商员工违反相关安全规定进行适当处罚，以保证项目安全无事故。以下具体处罚标准经XX与承包商协商同意，XX有权视项目安全管理情况要求承包商执行XX处罚标准，承包商有权为了自身加强项目安全管理提高处罚标准。
细则包括，但不限于：
处罚
1． 达到或超过2米作业未系安全带，处罚100元（RMB，下同）
2． 进入施工现场未戴安全帽（含未系下腭带）处罚50元
3． 未戴或未正确偑戴安全防护眼镜 处罚10元
4． 未穿安全鞋 处罚50元
5． 应申请作业许可证但未申请并作业 处罚100元
6． 特殊工种无资质上岗作业 处罚100元
7． 非吸烟点吸烟 处罚100元
8． 未有作业方案，未有安全检查（记录）处罚100元
奖励
XX成都项目为更好地推动各承包商遵守安全操作要求，建立安全奖励制度。每两个星期作一次评比。设立安全名星奖，金奖，银奖，铜奖共三等。金奖奖励300元，银奖奖励200元，铜奖奖励100元。
评比方法：
1． 从自觉遵守XX安全规章制度的承包商人员中产生；
2． 带头、监督提醒工人严格遵守安全规章制度的人员中产生
XX定期审核承包商对细则的执行情况，如发现承包商对此处罚细则执行不力，XX有权对承包商的安全管理不力给予RMB1000—10000的处罚。
承包商应加强工地现场安全检查，并不断改进。要求每天至少执行全面安全巡检3次并作好记录。如果未能实施安全巡查，XX将视同承包商安全管理不力。对此进行上述规定的安全管理不力的处罚。
所有罚款归XX控制用于项目建设安全投入，或作为安全奖励专款。
1. 各违规项目之处分方式如下表所示：

第二级
Level 2 1. 影响AA公司形象，如:污染临近道路、破坏周遭公共设施、引起临近公司抱怨，废弃物乱丢弃等。
Event to harm the company image, such as polluting the neighboring
roads, damages to the surrounding public facilities, complaints by
neighboring companies, dumping wastes freely at any place.
2. 经政府机关单位开立罚单者。
Receive a penalty ticket from the government.
3. 造成AA财产损失(含影响生产)之事件。
Event to cause company property loss (including interference with
production).
4. 造成人员受伤之事件。
Accident to cause personal injury or casualties.
5. 对人员有恐吓威胁或攻击殴打之行为。

Level 3 1. 使用不符合标准之必要安全卫生设施。(如：安全网、开口防护、施工架、照明、钢瓶、灭火器等设备)。
Use of non-conforming but necessary sanitary facilities such as safety
net, open guards, fabrication support, illumination, steel bottle, fire
extinguishers, etc.
2. 使用不符合防护标准之机械器具。(如：电焊机外壳无接地、无防止电击装置、电线经潮湿路面、无漏电断路器等装置)。
Use of non-conforming machinery tools such as the outer shell of the
electric welding machine without grounding, no strike-prevention
device, wire line through wet and damp ground surface, or device of
no-leakage breaker
3. 使用未经检查合格之危险性机械或设备。
Use of non-conforming and failed dangerous machinery or
equipment.
4. 危险性机械或设备之操作人员无取得政府或政府委办机构核发之合格证照。
No license for the dangerous machinery or the operator issued by
government agency.
5. 高架作业〈超过2公尺〉未设安全围篱或未使用安全带。
Overhead operation (exceeding 2 meters) without safety fence or
without use of safety belt.
6. 工作场所饮用酒精性或亢奋性饮料。
Drinking alcohol or stimulant drinks on the job site.
7. 违反政府相关法令，情节明确者。
In violation of related government rules and regulations.
8. 违反AA相关作业规定。
In violation of related operating rules enforced at .
9. 违反吸烟管制。
In violation of smoking control rules.
10. 违反动火、吊挂、缺氧、局限空间、高架作业及活线作业规定。
In violation of hot work、hanging operation、suffocation、confined
space and overhead operation rules.
11. 未执行承包商协议组织要求事项。
Failure to implement requirement set by the coordination
organization.
12. 文书资料未如期缴交齐全。

Level 4 1. 承包商未依规定告知下包商工作内容、危害告知。
The contractor fails to inform its sub-contractor of the job
contents or the danger thereof.
2. 未保持施工场所清洁，施工完毕之废弃物未依厂内规定清理和分类。
Failure to keep the job site clean and failure to clean up or classify the
waste after completion of the job.
3. 不出席承包商协议组织通知参加之各项会议 / 联合巡检。
Failure to attend to the various meetings and joint circuit inspections
as notified by the plant authority.
4. 未依规定申请暨配戴识别证，或未穿着识别背心或制服。
Failure to apply for and wear the ID card or failure to wear the ID vest
or uniform.
5. 未戴安全帽或安全帽之帽带未扣。
Failure to wear the safety helmet or to close the hat belt of the safety
helmet.
6. 施工架、电气机具、电盘等未标示、电盘未上锁。
No labels on the fabrication support, electric machinery/ tool, or
scaffold, the power box is not locked.
7. 使用未经承包商协议组织申请许可之电器机具。
Use of unauthorized electric machinery or tools.
8. 钢瓶未直立并固定。
Failure to put the steel bottle in a fixed upright position.
9. 未申请作业许可，未进行自动检查并保存记录。
Failure to apply for work permit implements self-inspection and keep
the records.
10. 施工区或物料堆放区未有标示或占用逃生动线。
Failure to post signs in the fabrication area or the stockpile storage
area; occupy emergency exit route.
