新生儿低血糖对神经发育的远期影响 阮毅燕 王金秋 陈瑜毅 冯军坛 沈开颜 蒙丹华 韦秋芬 摘要 目的:探讨新生儿低血糖对神经发育的远期影响。方法:对90例新生儿低血糖患儿进行随访,于 1岁时行听力、神经系统检查及Gesells评估,对随访资料进行回顾性分析。结果:(1)37.8%患儿有程度不同 的智力缺陷,33_3%有听力障碍,7.8%患继发性癫痫,3-3%患脑性瘫痪。(2)Gese11评估结果显示,新生儿 低血糖患儿中.有临床症状者其发育商(DQ)均低于无症状者;新生儿血糖最低值≤1.7 mmol/L,其DQ均 低于血糖>1.7 mmol/L者:低血糖持续时间>24 h者其DQ均较低血糖持续时间≤24 h者显著降低;除精 细动作外.头颅MRI检查异常者DQ显著低于头颅MRI正常者。结论:新生儿期严重长时间低血糖可导致 神经发育障碍及认知功能缺陷;低血糖早期头颅MRI检查结果对评估脑损伤及其远期预后有重要价值。 关键词新生儿: 低血糖; 认知功能; 神经发育 

Long term effect of neonatal hypoglycemia on nervous developmant in infants R UA N Yi—yah , WA NG Jin—qiu,CHEN Yu—yi,FENG Jun—tan,SHEN Kai—yon,MENG Dan—hua, Qiu-fen. Department of Peadiatrics,Maternal and Child Health Hospital of Guangxi,Nanning 530000,China 【Abstract】Objective To investigate the long—term e ̄cts of neonatal hypoglycemia on neurodevelopmental outcome in children. Method Ninety cases with neonata1 hypoglycemia were followed up to 1 year of age. Hearing,nervous system and Gesell assessment were performed in all subjects.The follow—up data was analyzed retrospectively.Results (1)About 37.8%cases were found to be mentally deficient,33-3%cases were hearing impairments.7.8%cases were suffered from epilepsy and 3.3%cases were suffered from cerebral palsy.(2)The results of Gesell assessment showed that the developmental quotient(DQ) in the cases with symptomatic hypoglvrcemia were significantly lower than those with asymptomatic subjects.The Des in cases with a minimal blood glucose level≤1.7 mmol/L in neonatal period were significantly lower than those of the glucose>1.7 mmol/L
