280例肢体恶性黑色素瘤临床分析

恶性黑色素瘤是一类来源于黑色素细胞的恶性 程度高、治愈难度大的恶性肿瘤，易沿淋巴系统和血 液系统播散、转移，危及患者生命[ 1 ] 。国内外相关文献 报道皮肤恶性黑色素瘤的患病率在有色人种与白种 人群中有较大的差异 [ 1 ] ，四肢发病多为肢端皮肤，近 12 年随着就诊病例的增多，系统、规范化、个体化的治 疗明显改善患者的总体临床受益率[ 2 ] 。现就对我院诊 治的 280 例病理确诊的皮肤恶性黑色素瘤的病历资 料进行回顾性分析。
1 资料与方法
1.1 临床资料 我院就诊的 280 例肢体恶性黑色素 瘤患者，均为得到我院病理确诊、有 HE 染色切片可 用，收治住院，具有完整的临床资料；其中接受我科室 系统、规范化治疗，有可随访信息病例 146 例。280 例 患者中，男性 154 例、女性 126 例（1 例发病期间有生 育史）；年龄 9-81 岁，平均 49.6 岁；上肢 108 例、下肢
高、治愈难度大的恶性肿瘤。该病起病隐匿，加之人们对其认识不到位，经常在肿瘤进展后来院就诊，预后较差。依靠病理学早期明确
诊断、及早手术、系统、规范化、个体化的治疗可延长生存期、提高生存率。

11 月就诊的四肢恶性黑色素瘤 280 例患者的临床资料，对其病历资料完整、接受系统治疗的 146 例病例、结合相关文献进行总结、分
析。结果：回顾性资料分析显示：原发于肢体的恶性黑色素瘤 95%发生于其末端，32%原发部位有外伤史、历经 5～20 年（平均 16 年），
后被活检病理证实。中位随访 39.9 月，1 年生存率 99%，3 年生存率 58%。结论：恶性黑色素瘤是一类来源于黑色素细胞的恶性程度
【Abstract】 Objective：The purpose of this study is to analyze the clinical and pathological features，treatment modalities for limb malignant melanoma. Methods: From 1998 to Nov 2009，clinical data in 280 patients with limb malignant melanoma among which 146 cases had complete clinicopathologic data and recepted the systemic treatment in our center was analyzed. Results: The result of data were retrospectively analyzed：primary limb malignant melanoma of occurs acral accounted for 95% among which the cases accounted for 32% had hurted in the same location from 5 to 20 years（average 16 years），who was diagnosed with pathological sections.With a median follow-up 39.9 months ，the one -year survival rate was 99% ，and three -year survival rate was 58% . Conclusion: Malignant melanoma is derived from melanocytic malignancy,and is refractory disease of highly malignant. Many people are not awareness of the disease because of its occult，and the patient's treatment is ofen bad for the quick progress of the disease.Diagnosis of the disease depends on pathology in which the standard，combination and individualization treatment could prolong the survival time and improve survival rate.
