兔抗人胸腺细胞免疫球蛋白【药品名称】通用名称:兔抗人胸腺细胞免疫球蛋白英文名称:Rabbit Anti-human Thymocyte Immunoglobulin【成份】本品主要成分:兔抗人胸腺细胞球蛋白每小瓶5ml含25mg兔抗人胸腺细胞球蛋白辅料:甘氨酸、氯化钠、甘露醇。
注射用水。
【适应症】1.移植用免疫抑制剂:预防和治疗器官排异反应。
治疗激素耐受和移植物抗宿主病(GvHD)。
2.血液学疾病:治疗再生障碍性贫血。
【用法用量】用量根据不同的适应症选择剂量,建议参考剂量为:器官移植的免疫抑制治疗:预防器官排异:肾脏、胰腺、肝脏移植后1.25-2.5毫克/kg/日,1至3周,心脏移植后3-10日。
终止用药时,无需逐渐减量。
治疗器官排异:2.5-5毫克/kg/日,至临床症状消失和生物学指标改善。
再生障碍性贫血:2.5-5毫克/kg日,连续5日。
此产品用于治疗再障的临床实验暂缺。
激素耐受和急性移植物抗宿主病治疗:剂量【不良反应】滴注兔抗人胸腺细胞免疫球蛋白时和用后有如下副反应的报道:全身性副反应为寒战、发热、心跳过速、呕吐和呼吸困难。
局部副反应有输液处局部疼痛及末梢血栓性静脉炎的报道。
罕见迟发性过敏反应,如初次使用后7-15日,可能会发生血清病(发热、搔痒、皮疹伴有关节痛)。
速发严重过敏反应极为罕见。
常见和极严重的副反应发生在第一次滴注后。
有些副反应的发生机理是与细胞分裂释放有关。
应用皮质醇和抗组胺制剂进行预防治疗并减慢滴速或增加稀释液量(等渗0.9%生理盐水或5%葡萄糖溶液)可降低或减轻【禁忌】急性感染时,禁用免疫抑制治疗。
对兔蛋白或本品其他成分过敏者。

4.2. 将切片置于以下染色缸中梯度反应:二甲苯10分钟×2次二甲苯5分钟无水乙醇5分钟×2次95%乙醇5分钟85%乙醇5分钟PBS清洗3分钟×3次4.3. 抗原修复(方法见一抗供应商,或采用常规高压、微波、酶修复等)4.4.根据组织大小直接滴加内源性过氧化物酶阻断剂到组织上,通常为100uL, 直至完全覆盖组织。
室温10分钟,PBS清洗3分钟×3次。
此步骤通常在水浴法、酶法等温和抗原修复后加入。

将上述两种溶液在56℃等量混匀,倾入夹有80mm×80mm×1.5mm“U”形模框的两块玻板中,待凝固后,于板之一侧打一排加样孔,孔径2mm,孔距3mm。
(2)将溶解后的标准血浆(试剂B)用水作1∶2、1∶4、1∶8、1∶16的稀释,样品也用水作1∶5稀释,然后用微量加样器分别定量加入加样孔,每孔5μl,于15V/cm电势梯度下,电泳6h(加样孔应置于负极侧)。
(3)电泳完毕,将凝胶板置4.4mmol/L磷钼酸溶液中,固定15min,量取各火箭峰,自加样中心至峰尖的高度(mm)。
以不同稀释度标准血浆的峰高(X)对相应的AT-Ⅲ抗原含量(Y)在正坐标纸上制作标准曲线。

多发性获得性易栓症的实验诊断摘要目的对多发性肠系膜静脉栓塞患者及其家系进行相关检查和基因诊断,分析其所患易栓症类型以及与遗传的相关性。
方法对该家系7人用发色底物法对蛋白C活性(PC:A)、抗凝血酶活性(AT:A)和蛋白S活性(PS:A)分别进行检测,用Western blot 检测血浆中的抗凝血酶含量和分子量,用Trizol法提取患者及家系成员外周血全基因组DNA,对抗凝血酶的7个外显子及其侧翼序列使用PCR扩增,用直接测序法对患者的AT扩增产物进行测序分析和基因突变分析。
结果该患者(II2)的AT含量正常, AT:A为49%,经AT基因序列的突变分析未发现突变位点,其他检测结果正常;患者弟弟(II3)AT:A为45%,其他检查结果正常。
该家系其他成员的PC:A 、PS:A 、AT:A及AT含量均正常。
结论根据易栓症相关检测结果及基因序列突变分析,患者与其弟弟有车祸外伤史(分别为髂骨骨折和肋骨骨折)等获得性诱因存在,该两患者不存在AT基因突变,诊断为获得性易栓症。

Background:The protein encoded by this gene is a plasma protease inhibitor and a member of the serpin superfamily.This protein inhibits thrombin as well as other activated serine proteases of the coagulation system,and it regulates the blood coagulation cascade.The protein includes two functional domains:the heparin binding-domain at the N-terminus of the mature protein, and the reactive site domain at the C-terminus.The inhibitory activity is enhanced by the presence of heparin.More than120mutations have been identified for this gene,many of which are known to cause antithrombin-III deficiency.[provided by RefSeq,Jul2009].
