第二次重修（重考）申请表Application Form for Second Examination RetakePlease note:1.According to the examination retaking rules set by school management, thosewho have not pass their respective compulsory courses and major electivecourses have to re-attend the course and retake the exam. Exam retake for each course should not exceed two times in total (except those who did not attend the exam or cheated). Under normal circumstances, the exam retake will be held in the first two weeks of the following semester.2.After exam retake, candidates could check their result from the respective school,department or via the online platform to register their result.3.Application period for second examination retake will be during the mid of May(for Spring semester), and mid of November (for Fall semester) every year.Candidates are required to apply according to their respective courses and hand in the application form to the teacher in charge of their respective batch within the application period. 备注：1.学生应在开学后三周内提出课程重（补）修申请，由任课教师、课程所属部门签署审批意见，经批准后并入相应班级跟读。

2.在期末考试前四周（第16周），由二级学院统一收齐申请表及相关学习证明材料后交至教务处审核，履行完手续后方可安排考试。

3.考试违纪作弊，在受到处分至少1年后，方可申请重修。

4.复学、转专业学生，需对照人才培养方案核实所缺课程完成补修。
