EDTA依赖性假性血小板减少标签：EDTA依赖性假性血小板减少症；EDTA随着全自动血细胞分析仪的广泛应用，乙二胺四乙酸盐(EDTA)因其对血细胞影响较小，在临床得到广泛使用。
但国外文献1980年已有报道表明，EDTA 可引起血小板聚集、黏附，导致EDTA依赖性假性血小板减少症(EDTA-dependent pseudo thrombocytopenia，PTCP)发生。
PTCP是一种由EDTA诱导，发生于体外的非稳固性血小板凝聚，临床主要表现为重型假性血小板减少症，且无出血现象［1］。
PTCP产生的假性血小板计数减少会导致患者临床辅助检查增多,严重时易导致临床错误诊治。
因此，PTCP应该在临床检验工作中得到广泛重视。
1资料与方法1.1一般资料回顾性分析30例临床检验过程中发生EDTA依赖性假性血小板减少症患者的门诊或住院资料，其中男17例，女13例，年龄20~68岁，平均(41.6 18.3)岁。
1.2检验方法笔者所在医院检验科采用多种方法为患者进行血小板计数检测，以避免计数结果错误。
（1)全自动细胞分析仪法:使用EDTA盐、枸橼酸钠抗凝管分别采集患者外周静脉血2.0 ml，采血后0、10、30、60、120 min分别用全自动血细胞分析仪及配套试剂进行血常规检测。
（2）手工计数法:根据《全血临床检验操作规程》第3版规程行手工计数,0.38 ml许汝和氏稀释液中置入患者指血20 μl，充分混匀后行血小板显微镜计数。
（3）血涂片瑞氏-姬姆萨染色法:使用EDTA盐、枸橼酸钠抗凝管分别采集患者外周静脉血，在采血后0及60 min 后各涂片1张,常规晾干后瑞氏-姬姆萨染色,显微镜下行血小板计数。
2结果2.1全自动细胞分析仪法不同时间点血小板计数结果EDTA管血小板计数在（10~142）×109/L，随患者血液标本抽取时间的延长而持续下降，10 min后已显著降至正常值以下；而枸橼酸钠管血小板计数则则无明显下降。

方法根据该实验室复检标准，采用血小板出现假性减少的50例门诊或住院患者作为研究对象，分析假性减少的原因及其复查方法。
结果50例 血小板假性减少样本中，42例为乙二胺四乙酸依赖性，3例为大血小板，2例白细胞周围卫星现象，3例冷凝集现象, 对这些患者进行复查后可得到正常范围的血小板结果。
结论E D T A-P T C P患者经镜检涂片发现聚集现象，可用枸 橼酸钠抗凝、无抗凝剂即刻上机、手工计数复查血小板数值以避免误诊。
