1、变质alteration
炎症局部组织发生的各种变性、坏死统称为 变质。其形态学变化有：
(1)实质细胞：细胞水肿、脂肪变性、凝固性坏死、 液化性坏死等。
(2)间质细胞：结缔组织的粘液样变性、纤维素样 坏死等。
原因：致炎因子的直接损伤、血液循环障碍、 炎症反应产物。
2、渗 出exudation
是指炎症局部组织血管内的液体成分、 纤维蛋白原等蛋白质和各种炎症细胞通 过血管壁进入组织、体腔、体表和粘膜 表面的过程。其中渗出的液体成分称为 渗出液；细胞成分称为炎细胞；两者合 称渗出物。
渗出为炎症最具特征性的变化，在局 部发挥着重要的防御作用。

第二节 炎 症 介 质
炎症介质（inflammatory mediator）是 指参与并诱导炎症发生的具有生物活性的化 学物质，也称化学介质。分为外源性（细菌 及其代谢产物）和内源性（细胞及血浆）两 大类，下面主要介绍内源性炎症介质。
一、细胞释放的炎症介质

(2)白细胞的游出机理
Why白细胞能穿过血管壁而游出到炎灶内？
白细胞游出是一种复杂的生物学现象,目前一般用
趋化学说(chemolaxis)来解释: 炎灶中存在某些化学
临床上见于慢性炎症、病毒感染及肿瘤边缘。
不等,直径6～30u,呈卵圆形或圆形。胞核较小而
圆，胞浆充满嗜碱性颗粒。

肥大细胞在形态上和分布上虽与嗜碱性粒细 胞有所不同,但他们所含的颗粒物质和功能都十 分相似。因此有人把二者统称为嗜碱肥大细胞 系统(basomastocyte system)
嗜碱性粒细胞和肥大细胞均存在有IgE和Fc受 体,能与IgE结合,带IgE的嗜碱性粒细胞和肥大细 胞与特异性抗原结合后,立即引起细胞脱颗粒 → 释放组织胺等介质 → 过敏反应---Ⅰ型变态反 应。多见于变态反应性炎。
胞,常常只见其核。
