CNAS—AL01实验室认可申请书Application for Laboratory Accreditation实验室名称：######检测有限公司##中心申请日期：2008 年 1 月10 日中国合格评定国家认可委员会二〇〇七年一月申请须知1、实验室在提交本《申请书》前应了解并自愿遵守中国合格评定国家认可委员会（CNAS）有关认可的政策和要求。

CNAS的公开文件可在CNAS 网站（）查阅。

2、当实验室申请校准项目认可时，应符合计量法的有关要求。

3、本《申请书》“随本申请书提交的文件资料”栏目中所提及的“典型项目的检测报告/校准证书及不确定度评估报告”，是指提交每一申请认可的领域中具有代表性的检测报告/校准证书，以及与所提交的检测报告/校准证书相对应的不确定度评估报告。

4、CNAS对申报资料进行审查，当审查结果为“暂缓实施现场评审”时，实验室在完善质量管理体系达到规定要求后，需运行3个月以上，方可安排现场评审。

5、如实验室需同时进行国家级计量认证、审查认可（验收）评审时，请从CNAS网站下载计量认证/审查认可（验收）申请书。

6、实验室递交本《申请书》的同时，应交申请费（人民币：600元）。

对港澳台及国外实验室的相关认可收费标准，依据国际惯例，由双方协商并在合同中约定。

申请费可汇入：请在汇款后将汇款单传真至：。

传真中须注明汇款实验室名称（当汇款单位名称与申请书中实验室名称不一致时，须注明申请书中的实验室名称）、联系人、地址、邮政编码、联系电话。

7、实验室须提交2份本《申请书》（含附表内容）书面文本和1份电子版本的本《申请书》，随本《申请书》提交的文件资料在申请时可只提交1套。

同时申请国家级计量认证、审查认可（验收）时，还应提交计量认证/审查认可（验收）申请书一份。

填表说明1．本《申请书》用计算机打印，要字迹清楚。

2．本《申请书》书面文本有关项目填写页数不够时可用A4纸附页，但须连同正页编第页，共页。
