AbstractObjectives:Theobjectivewastodeterminethediagnosticaccuracyofprocalcitoninmeasurementforbacterialinfectionsinpatientswithallcausesoflivercirrhosis.Methods:Theauthorsconductedacross-sectionalstudyof98patientswithcirrhosistreatedintheemergencydepartment(ED)ofChang-GungMemorialHospital,Taiwan.Serumprocalcitoninlevelsandotherclinicalinformationwereobtainedconcurrently.Patientswereassignedtoasepsisornonsepsisgroupafterthemedicalrecordswerereviewedbytwoemergencyphysiciansblindedtothestudy.Receiveroperatingcharacteristic(ROC)curveanalysiswasconductedtodeterminethesensitivity,specificity,likelihoodratio,andsuggestedcutoffvalues.ThediagnosticaccuracyoftheC-reactiveprotein(CRP)levelwasalsodeterminedforcomparison.Results:Atotalof98patientswereenrolledforanalysisin1year.Twenty-sevenpatients(27.6%)wereassignedtothesepsisgroup.Elevenpatients(11.2%)hadpositivebloodcultures.TheareasundertheROCcurvesforprocalcitoninandCRPinpredictingsepsiswere0.89(95%confidenceinterval[CI]=0.77to0.92)and0.81(95%CI=0.72to0.89),respectively(p=0.11).ThecutoffthatmaximizedYouden’sindexwas0.49ng⁄mLforprocalcitoninand24.7mg⁄LforCRP.Atthesecutoffs,thesensitivityandspecificitywere81.5and87.3%forprocalcitoninand80.0and80.3%forCRP.TheseresultssuggestthatprocalcitoninmeasurementshowsatleastanequivalentdiagnosticaccuracytoCRPmeasurement.Conclusions:ProcalcitoninprovidedsatisfactorydiagnosticaccuracyindifferentiatingbacterialinfectionsinpatientswithallcausesoflivercirrhosisintheED.Acutoffvalueof0.5ng⁄mLissuggestedforclinicaluse.ACADEMICEMERGENCYMEDICINE2011;18:122–126ª2011bytheSocietyforAcademicEmergencyMedicine
