第一栏：应填写中国出口商的法人全称、地址（包括国家）。

第二栏：应填写新加坡收货人的法人全称、地址（包括国家）。

第三栏：应填写运输方式、路线，并详细说明离港日期、运输工具的编号及卸货口岸。

第四栏：由进口方海关在相应的栏目内标注（√）。

第五栏：应填写项目号码。

第六栏：应填写唛头及包装号码。

第七栏：应详细明包装数量及种类。

详细列明每种货物的商品描述，以便于海关查验时识别。

商品描述应与发票所述及《协调制度》的商品描述相符。

如果是散装货，应注明“散装”。

第七栏的每种货物应填写《协调制度》八位或十位数编码。

第八栏：若货物符合原产地规则，出口商必须按照下列表格中规定的格式，标明其货物享受优惠关税待遇所依据在原产地标准：
第九栏：毛重应填写“千克”。

可依惯例填写其他计量，如体积、数量等，以准确反映其数量。

FOB价格应在此栏中注明。

第十栏：应填写发票号码及发票的日期。

第十一栏：出口商授权的手签人填写、并盖章。

第十二栏：本栏由签证机构签名、盖章。

RCEP原产地证书填制说明第1栏：货物运自（出口商名称、地址、国家）此栏应填写已办理原产地证书申请人备案的中国境内1出口商的名称、详细地址和国家。

此栏不得填写两个或两个以上公司名称。

第2栏：货物运至（进口商/收货人名称、地址、国家）此栏应填写RCEP成员方收货人名称、地址和国家。

此栏不得填写非进口方公司信息。

第3栏：生产商名称、地址、国家（如已知）此栏应填写货物生产商的名称、地址和国家。

如果证书货物由多个生产商生产，此栏填写“SEE BOX 8”，同时应在第八栏每项货物描述后列明对应生产商的名称、地址、国家；出口商或生产商希望该信息保密的，可在该栏填写“CONFIDENTIAL”；如果不知道生产商，可填写“NOT A V AILABLE”。

第4栏：运输工具及路线（如已知）此栏应填写离港日期、船舶名称/航班号等和卸货口岸。

出运后申报的原产地证书，此栏必须填写具体运输工具名称及号码，出运前申报且具体运输工具名称及号码未知的原产地证书，此栏可填写“***”或“BY SEA”或“BY AIR”或其它运输方式。

装货口岸应为中国境内口岸，卸货口岸应为RCEP成员方口岸。

1本说明中，中国境内不包含港澳台地区。

由于证书栏目空间有限，不再打印“FROM…TO…”的运输细节。

第5栏：官方使用此栏留空。

第6栏：项目编号在收货人、运输条件相同的情况下，如同批出口货物有不同品种，则可按不同品种分列“1”、“2”、“3”……，以此类推。

第7栏：包装唛头及编号此栏填写的唛头及包装号应与发票、货物外包装上的一致。

如货物没有唛头及包装号应填“N/M ”或“NO MARK”或“NO MARKS AND NUMBERS”。

如有特殊唛头的，可在此栏填写“SEE ATTACHMENT”，并在证书背页贴唛（盖骑缝章）或A4 白纸打印唛头（盖骑缝章）。

CARTON LABEL、AS ADDRESS、AS PER INVOICE、AS PACKING LIST、AS B/L、AS BILL OF LADING、COLOR LABEL 等不能作唛头。

中国新加坡自由贸易协定产品特定原产地规则中国和新加坡自由贸易协定产生于2024年，Major械是在2024年11月签署的；新加坡和中国自由贸易协定的原产地规则是指好向受该协定影响的国家的商品可以使用该协定给予的贸易优惠，必须满足该协定的原产地条件。

双原产地规则对于中国新加坡原产的商品来说，当需要满足的规定中包含“微小细微改动”（minor alterations）的条件时，只要经过以下任一情形中的一种处理，即可获得中国新加坡原产地状态：
一种处理是：原产地在中国的处理成分经新加坡组成了最终产品的上料所需，则可以获得双原产地资格；
第二种处理是：最终产品经过新加坡的装配过程，则可以获得双原产地资格；
第三种处理是：最终产品至少有新加坡30%的成份，则可以获得双原产地资格。

中国—东盟自由贸易区优惠原产地证明书(FORM E)填制说明签发中国—东盟自由贸易区优惠原产地证书的国家和代码：96文莱 Brunei、116 柬埔寨 Cambodia、360 印尼 Indonesia、418 老挝 Laos,PDR、458 马来西亚 Malaysia、104 缅甸 Myanmar、608 菲律宾 Philippines、702 新加坡Singapore、764 泰国 Thailand、704 越南 Vietnam，给予关税优惠的商品为HS编码中1-8章的农副产品，该项下的产品享受零关税待遇。

1、证书号：证书标题栏（右上角），填上检验检疫机构编定的证书号。

证书号编定规则 E083333331210001E-中国——东盟自由贸易区优惠原产地证明书 08-2008年 333333121-公司注册号 0001-企业流水号2、第1栏：出口商名称、地址、国家此栏出口商公司名称应与注册时相同。

必须打上国名、地址。

例：ZHEJIANG NATIVE PRODUCE & ANIMAL BY-PRODUCTS I/E CORP.NO.368 NORTH ZHONGSHAN ROAD，HANGZHOU，CHINA3、第2栏：收货人的名称、地址、国家此栏须填上给惠国最终收货人名称，不可填中间转口商的名称,此栏须打上国名。

4、第3栏：运输细节根据证书要求如实填写出运日期、运输工具名称和装卸货港口5、第4栏：官方使用不论是否给予优惠待遇，进口成员国海关必须在第4栏作出相应的标注。

6、第5栏：商品顺序号如同批出口货物有不同品种，则按不同品种分列“1”、“2”、“3”……，以此类推。

单项商品，此栏填“1”。

7、第6栏：唛头及包装号填具的唛头应与货物外包装上的唛头及发票的唛头一致；唛头不得出现中国以外的地区和国家制造的字样，也不能出现香港、澳门、台湾原原产地字样（例如：MADE IN TAIWAN，HONG KONG PRODUCTS等）；如货物无唛头应填“N/M”。
