1 对象与方法
1. 1 对象
研究在上海洋泾、凌云和华阳三个街道中进行。 1. 2 方法
2004年 5 - 6 月, 研究小组分赴前述各街道开 展了现场考察与调研。根据上述现场调研所掌握信 息, 编制 社区居民调查问卷 。问卷由自行编制的 一般资料调查表、一般健康问卷 ( GHQ ) [ 4] 组成。

2 结果
2. 1 被调查者家庭情况 不同街道有效答卷者在年龄、文化程度和职业
上存在显著差异 [ 3] , 各街道被调查者家庭情况的差 异较大 (表 1) , 部分反映出三个社区自然聚集的居 住人口的构成不同。华阳街道居民收入较高; 洋泾

问卷通过街道办事处向社区居民发放和回收。 共发放居民问卷 4050份 ( 3 个街道 86个居委会各 发放 50份 ) , 收回问卷 3675份, 其中有效答卷 1317 份 ( 占发放问卷的 32. 5% )。
2004年 6 月 - 7 月对收 回的问 卷采 用 SPSS / PC9. 0软件包进行统计分析, 各项目均按实际有效 回答作出分析。

论著
精神卫生问题是全球性的重大公共卫生问题和 较为严重的社会问题。我国目前约有重性精神疾病 患者 1600万人, 神经精神疾病约占我国疾病总负担 的 20% , 排名首位。此外, 受到情绪障碍和行为问 题困扰的 17岁以下儿童和青少年约 3000万。根据 世界卫生组织推算, 中国神经精神疾病负担到 2020 年将占到疾病总负担的四分之一以上 [ 1, 2] 。

费负担较重 ( 占家庭收入 31% 以上 ); 享受普通医保 和大病医保者比例较高的是华阳街道, 洋泾街道被 调查者享受贫困救助和自费的比例较其他两地高。 尽管综合性医院是各街道居民就医的主要途径, 但 相对来说, 华阳街道居民选择社区卫生服务中心的 比例较高、凌云街道居民选择二级专科医院的比例 较高; 华阳街道的被调查者家人中有慢性躯体疾病、 精神分裂症和焦虑症患者的比例也高于其他两地。 2. 2 被调查者心理问题的发生率和影响因素分析

Abstract O bjective: T he study aim ed to investigate m en tal health status in Shangha i residen ts, and thus to prov ide inform ation for policy - m ak ing and stra tegy of m enta l hea lth wo rk. M e thods: Based on fie ld study, the questionna ire for survey w as developed. The study w as conducted in three communities in Shangha,i and 1317 residents finished the questionna ires. R esu lts: 15. 9% o f the investigated residen ts probab ly had m ental hea lth problem s. T he risk factors for deve loping m enta l hea lth prob lem s are low er education, unemp loym ent, be ing la id- of,f the retired, poverty and fam ily m ember of the menta lly d iso rdered. P eop le w ith m ental problem s are m ore likely to seek he lp in comm un ity ho spita l and genera l hosp ita.l Con clusion: To streng then and e fficien tly carry ou t the m en tal health serv ice in commun ity and general hosp ital is the long term goa l and task for m enta l health wo rkers.
