老年COPD患者急性加重期血浆D—D、TAT和PAP含量的检测及意义作者：蒲艳来源：《中国社区医师》2015年第01期摘要目的：老年COPD患者的急性加重期患者的血浆D-D、TAT及PAP含量明显较高，容易导致患者的血栓形成，出现弥散性血管内凝血，导致患者死亡，在临床当中联合检测患者的D-D、TAT及PAP可以有效地掌握患者的临床情况及治疗效果。

本次研究主要针对老年COPD患者急性加重期的临床检测，结合文献资料，研究血浆D-D、TAT及PAP在患者病症中的表达，为临床治疗提供一定的参考。

关键词血浆D-D； TAT； PAP；老年COPD患者；急性加重期老年COPD患者的急性加重期患者的临床危险性较高，在临床上的死亡率较高，在临床检查的过程中患者的血栓因素致死，要想更好地了解掌握患者临床病情及隐藏病症，就要在治疗中注意对血浆情况进行检查。

COPD急性加重期COPD是慢性阻塞性肺病，是可以通过预防和治疗的疾病，具有气流受限特征，气流受限情况不完全可逆、且进行性地进展，它与呼吸道对烟雾香薰等有害气体或有害颗粒的敏感异常炎性反应有关。

而老年COPD急性加重期主要表现为呼吸困难加重，并伴有乏力、胸闷、气喘、发热，咳嗽加重、痰量增多、痰液颜色和性状改变等，还有可嗜睡、全身不适、精神紊乱、抑郁不安，失眠等表现。

老年COPD急性期往往合并有肺部感染、Ⅱ型呼衰、心功能不全，病情较危重，临床危险性也较高，如果不能适时地处理治疗，很容易造成患者临床病症加重或出现其他并发症，影响患者的生命安全。

COPD是近年来国内外十分重视的慢性呼吸系统疾病之一。

该病具有一组异质性的临床现象，其中包括慢性支气管炎、肺气肿以及哮喘合并慢性支气管炎，不少患者最终发展为慢性呼吸衰竭及慢性肺源性心脏病，严重危害患者的健康。

慢性阻塞性肺病是老年人常见慢性呼吸道疾病，该病不但造成对老年患者活质量的影响，还使得老年人活动能力受限，并对老年人造成较高的病死率。

慢性阻塞性肺疾病急性加重期凝血功能检测的临床意义潘君素　张茵　郑静(浙江省台州市立医院呼吸科,台州　318000)【摘要】　目的　探讨慢性阻塞性肺疾病(COP D)急性加重期凝血功能的变化。

方法　对78例C OP D急性加重期患者、55例COP D稳定期患者和20例健康体检者检测4项凝血(PT、APTT、TT、Fbg)和D-二聚体,用SPSS1410软件进行单因素方差分析。

结果　急性加重期患者Fbg和D-二聚体含量显著高于稳定期患者和正常对照组,稳定期含量也明显高于正常对照组(P<0105),PT、APTT、TT含量在三组间差异无统计学意义(P>0105)。

结论　对C OP D患者进行监测Fbg和D-二聚体水平,有助于采取有效的防治措施,可控制病情的发展,改善疾病的预后。

Methods 69 patients with acute exacerbation of COPD(AECOPD) which achieved remission during and after hospital treatment were selec ted. Double-antibody sandwich enzyme linked immunosorbent assay was employed to measure the concentration of serum SP-D,and the complete blood count test was performed simultaneously. 42 cases of healthy people accepted physical examine served as control. Results Average concentration of serum SP-D in patients with AECOPD during acute exacerbation and in stable phase were (183 + 86) ,( 126 + 72) ng/mL, respectively, while that in control group[( 96 ± 47) ng/mL]. There were statistically significant differences between patients with AECOPD and patients in stable phase or in control group(P<0. 01). The positive rate of serum SP-DD in patients with AECOPD were 85. 50% which demonstrated statistical difference when compared with those of WBC and neutrophils(P<0. 01). 方法选取2016年2月至2017年10月在我院接受治疗的COPD老年患者，其中急性期COPD患者42例作为A组,35例稳定期COPD老年患者作为B组，同期30例健康老年人作为C组，采用ELISA检测P物质水平，计算COPD病情及预后预测指标BODE指数得分，分析两者的相关性。

结果COPD急性期和稳定期患者P物质水平均高于健康老年人(P<0.05),且急性期患者高于稳定期患者(P<0.05)；A组和B组患者血浆P物质水平均与BODE指数得分成正相关(P<0.05)o结论通过检验老年COPD急性发作期患者血浆P物质水平可以较准确地判断病情和预后，对临床诊断和治疗提供了一定指导。
