Preeclampsia developed in165patients.Bottom Line:Neither first-trimester serologic screening nor second-trimester uterine artery Doppler velocimetry were clinically useful for predicting preeclampsia in these low-risk populations.I also would go one step further and state that,without some proven,effective intervention beyond what we already have(clinical judg-ment!)implementing a screening test for preeclampsia is of minimal to no use.Clinical Management of Severe Preeclampsia Remote From TermThe management of severe preeclampsia before34 weeks of gestation has been a subject of study for more than2decades.The management decisions must weigh the risks of prematurity to the neonate against the risks to the mother of delaying delivery.Because maternal mortality in developed countries is fortunately a rare occurrence,much of the evidence to support manage-ment comes from case series,retrospective analyses, small clinical trials or clinical opinion.This relative paucity of level I evidence led the Society for Maternal-Fetal Medicine to offer guidelines for the evaluation and management of severe preeclampsia before34weeks of gestation based on a MEDLINE review of all relevant published peer-reviewed articles between January1980 and December2010,and relied on expert commentary an opinion where reliable data were not available. Bottom Line:There are several areas of level I–II or level A evidence in this area,including:1.Expectant management of severe preeclampsiaremote from term is appropriate in selected cases, with noted improvement in newborn outcomes and without apparent added maternal risk.2.Antenatal steroid administration is recommen-ded before delivery regardless of whether or not expectant management is being considered.3.Severe proteinuria alone or degree of change inproteinuria should not be used as a criterion for delivery.4.Hemolysis,elevated liver enzymes,and low plateletcount syndrome should not be managed expectantly.Long-Term Sequelae of PreeclampsiaThe study by Aukes et al is from The Netherlands and investigated73formerly preeclamptic women and matched controls for the presence of white matter lesions on cerebral magnetic resonance imag-ing.Patients with a history of preeclampsia had a higher rate of white matter lesions than did controls,and persistent hypertension and early onset preeclampsia were independently associated with having white matter lesions.In another study by Wiegman et al,also from The Netherlands and including some of the same investi-gators as Aukes et al,47formerly eclamptic women and 47controls were evaluated for vision-related quality of life.The investigators noted that formerly eclamptic women had a lower vision-related quality of life,and that this reduction also seemed to be associated with the presence of cerebral white matter lesions.Finally,Smith et al studied a longitudinal cohort of patients with preeclampsia.These Canadian investiga-tors monitored the patients for1year and then calculated each individual’s10-year,30-year,and life-time risk of cardiovascular disease.The10-year, 30-year,and lifetime risk for cardiovascular-disease development was18.2%compared with1.7%,31.3% compared with5.1%,and41.4%compared with17.8%, respectively,with the preeclamptic patients risk being higher than the controls in each category and with sta-tistically significant odds ratios in all categories. Bottom Line:Preeclampsia may be a risk factor for permanent cerebrovascular damage.In addition, there is growing evidence that preeclampsia may be a marker for long-term health risks,and that patients with a history of preeclampsia should be strongly encouraged to remain engaged in preventive health care,much as we recommend for all patients but most especially for other high-risk groups.My own final editorial comment is that,perhaps we should start thinking of magnesium sulfate as being “neuroprotective”for not only the fetus but perhaps for the mother as well,and that universal use of magne-sium sulfate even in mild preeclampsia may have ben-efits that go beyond only seizure prophylaxis.VOL.121,NO.3,MARCH2013Repke What’s New in Preeclampsia683。

What parts does a news story have?
•All news stories answer some basic questions: who, what, when, where, why and how? •Like all stories, the basic news story has a headline and three general parts: a beginning called the “lead”, a middle called the “body” and an ending.
• A news story is composed of headline, lead, body, and before lead, there are dateline and byline.
•Headline: The headline is the line on top of the story that tells the reader what the story is about.
•Lead: the beginning of the story. It tells the reader what the story is about.
• A good lead attracts the reader to continue reading.
•In a hard-news story, the lead usually is written in one sentence—the first sentence of the story—and gives the most important information about the event.
•The dateline, which tells readers when and where the story is written, is important.
•The byline, which tells us the writer, is also very important.
Categories of News
•“Hard news” :
stories of a timely nature about events or conflicts that have just happened or are about to happen, such as crimes, fires, meetings, protests, speeches and so on.
•These stories have immediacy.
“Soft news” :
•news that entertains or informs, with an emphasis on human interest(人情味) and novelty(新
奇)and less immediacy than hard news.
For example, stories about bad effects of smoking, of drug abuse, etc. would be considered soft news.
“Soft news” can also be stories that focus on people, places or issues that affect readers’ lives. These types of stories are called “feature stories.”
It isn’t less important than hard news, but it isn’t news that happened overnight.

•What are the qualities of a good news story?

Some events that happened in the past also may be considered timely if they are printed on an anniversary of the event, such as one, five or 10 years after the incident.
Proximity
•An event may be of interest to local readers because it happened in or close to the community.
Unusual Nature: the uniqueness of an event
•There is a well-known saying:

•People who are well-known for their accomplishments—primarily entertainers,athletes or people who have gained fame for achievements, good or bad—attract a lot of attention.
Human interest:
•People like stories about people who have special problems, achievements or experiences. •These stories can be profiles or unusual stories about people that make readers care about their lives.
Conflict & Impact:
•Stories involving conflicts people have with government or other people are often newsworthy, especially when the conflict reflects local problems.
Additional qualities
•Helpfulness: Consumers, health and other how-to stories.
•Entertainment: Stories that amuse readers, make them feel good or help them enjoy their leisure time.
