二、适用范围:本指南适用于进口可用作原料的固体废物国内收货人注册登记、变更、延续、注销业务的办理。
三、事项审查类型:前审后批。

五、申请条件:(一)具有合法进口经营资质的加工利用企业;(二)具有固定的办公场所及相应基础设施,具备放射性检测设备及检测能力;(三)熟悉并遵守中国出入境检验检疫、环境保护、固体废物管理的法律法规及相关标准;(四)具有对废物原料进行环保质量控制及加工利用的措施和能力,并建立相应的管理制度,保证废物原料符合中国出入境检验检疫、环境保护、固体废物管理的国家技术规范的强制性要求。
六、禁止性要求:申请人的申请不符合法定条件、标准。
七、申请时应提交的材料:(一)首次申请。
申请人应当凭以下材料办理进口废物原料收货人注册登记:1.注册登记申请书;2.环保部门批准从事进口固体废物加工利用的证明文件。

二、适用范围:本项目适用于进境(过境)动植物及其产品检疫审批事项的申请和办理,具体包括以下品种:(一)进境活动物(含动物,胚胎、精液、受精卵、种蛋及其他动物遗传物质)、生物材料、非食用动物产品、饲料及饲料添加剂、果蔬类、烟草类、粮谷类、豆类、薯类等;(二)过境动物;(三)特许审批范围包括:动植物病原体(包括菌种、毒种等)、害虫以及其他有害生物,动植物疫情流行国家和地区的有关动植物、动植物产品和其他检疫物,动物尸体,土壤。

二、项目信息项目名称:出入境特殊物品卫生检疫审批项目编码:三、办理依据1、《中华人民共和国国境卫生检疫法》第四条规定:入境、出境的人员、交通工具、运输设备以及可能传播检疫传染病的行李、货物、邮包等物品,都应当接受检疫,经国境卫生检疫机关许可,方准入境或者出境。

直属检验检疫局对准予进出口的,出具《出入境特殊物品卫生检疫审批单》。

九、申请材料(一)申请特殊物品审批的,货主或者其代理人应当按照以下规定提供相应材料1.《入/出境特殊物品卫生检疫审批申请表》;2.出入境特殊物品描述性材料,包括特殊物品中英文名称、类别、成分、来源、用途、主要销售渠道、输出输入的国家或者地区、生产商等;3.入境人体血液、血浆、组织、器官、细胞、骨髓等,应当提供卫生主管部门的批准文件;4.入境、出境供移植用人体组织、细胞、器官、骨髓,应当提供医疗机构出具的供体健康证明和相关检验报告;5.入境用于预防、诊断、治疗人类疾病的生物制品、人体血液制品,应当提供国务院药品监督管理部门发给的进口药品注册证书;6.入境、出境特殊物品含有或者可能含有病原微生物的,应当提供病原微生物的学名(中文和拉丁文)、生物学特性的说明性文件(中英文对照件)以及生产经营者或者使用者具备相应生物安全防控水平的证明文件;7.出境用于预防、诊断、治疗的人类疾病的生物制品、人体血液制品,应当提供药品监督管理部门出具的销售证明;8.出境特殊物品涉及人类遗传资源管理范畴的,应当提供人类遗传资源管理部门出具的批准文件;9.使用含有或者可能含有病原微生物的出入境特殊物品的单位,应当提供与生物安全风险等级相适应的生物安全实验室资质证明,BSL-3级以上实验室必须获得国家认可机构的认可;10.出入境高致病性病原微生物菌(毒)种或者样本的,应当提供省级以上人民政府卫生主管部门的批准文件。

“出口食品生产企业备案核准”行政审批事项服务指南
一、行政审批事项名称:出口食品生产企业备案核准。
二、适用范围:本指南适用于中华人民共和国境内出口《实施出口食品生产企业备案的产品目录》内食品生产、加工、储存企业的备案工作,不包括出口食品添加剂、食品相关产品的生产、加工、储存企业。
三、事项审查类型:前审后批。
四、审批依据:
(一)《中华人民共和国食品安全法》(年月日第十一届全国人民代表大会常务委员会第七次会议通过,年月日第十二届全国人民代表大会常务委员会第十四次会议修订)。
(二)《中华人民共和国海关实施〈中华人民共和国行政许可法〉办法》(海关总署令第号公布,根据海关总署令第号修改)。
(三)《中国出口食品生产企业备案管理规定》(国家质量监督检验检疫总局令第号,根据海关总署令第号修改)。
五、申请条件:出口食品生产企业应当建立和实施以危害分析和预防控制措施为核心的食品安全卫生控制体系,该体系还应当包括食品防护计划。
出口食品生产企业应当保证食品安全卫生控制体系有效运行,确保出口食品生产、加工、储存过程持续符合我国相关法律法规和出口食品生产企业
安全卫生要求,以及进口国(地区)相关法律法规要求。
六、禁止性要求:对于已受理的行政许可申请,经审查,认为申请人不具备法律法规规定条件或者不符合法律法规规定要求的,不予许可。
七、申请时应提交的材料:
出口食品生产企业备案时,应当提交书面申请和以下相关文件、证明性材料,并对其备案材料的真实性负责:(一)首次申请
.出口食品生产企业备案申请书(首次申请);
(二)延续申请。
.出口食品生产企业备案申请书(延续备案);
(三)变更申请。
.变更申请。
(四)注销申请。

八、办理流程:
(一)许可的申请、受理、核查、决定。
.申请人向主管海关递交材料。
主管海关对申请人提交的备案材料进行初步审查,材料齐全并符合法定形式的,予以受理;材料不齐全或者不符合法定形式的,应当一次性告知出口食品生产企业需要补正的全部内容。
.主管海关应当在受理备案申请后,组织专家完成评审工作,出具专家评审报告,并作出是否准予备案的决定。
.准予备案的,向企业颁发《出口食品生产企业备案证明》(以下简称《备案证明》);不予备案的,应当书面告知申请人并说明理由。
(二)许可的延续。
在《备案证明》有效期届满日前,申请人向原发证海关书面提出延续申请。
原发证海关应当在《备案证明》有效期届满前做出是否准予延续的决定。
(三)许可的变更。
申请人应当自发生变更之日起日内,向原发证海关递交申请材料,原发证海关对申请变更内容进行审核。
变更申请材料齐全、说明材料真实有效的,准予变更。
(四)许可的注销。
申请人需要注销《备案证明》的,向主管海关提出书面申请,经主管海关审核后,办理注销手续。
九、办结时限:
主管海关自受理行政许可申请之日起个工作日内做出行政许可决定。
十、收费标准:不收费。
十一、审批决定法律文书及相关证件:总署文件。
