食管鳞癌组织中细胞周期素B1和D1表达的意义

【摘要】目的:探讨食管鳞癌组织中细胞周期素B1和D1(Cyclin B1, Cyclin D1)mRNA 和蛋白的表达及其与食管鳞癌床生物学指标的关系. 方法:应用原位杂交法、免疫组化SP 法对62例食管鳞癌组织(组织学Ⅰ级15例,Ⅱ级25例,Ⅲ级22例;有淋巴结转移20例,无淋巴结转移42例;肿瘤浸润深度:浸润至黏膜层、黏膜下层或浅肌层7例,深肌层14例,纤维膜41例)、31例癌旁不典型增生组织及62例正常食管黏膜组织进行Cyclin B1,D1 mRNA和蛋白的检测,分析其阳性表达与食管鳞癌患者临床病理因素的关系. 结果:Cyclin B1,D1的mRNA和蛋白在正常食管黏膜组织中不表达或低表达,在癌旁不典型增生组织及食管鳞癌组织中高表达,三者阳性表达率比较差异有统计学意义(P<0.05);食管鳞癌患者癌组织中CyclinB1,D1的mRNA和蛋白的表达与性别、年龄无关;与组织学分级、浸润深度及有无淋巴结转移有关(P<0.05). Cyclin B1,D1 mRNA与蛋白正常食管黏膜组织、癌旁不典型增生组织及食管鳞癌组织中的表达有高度的一致性. 结论:Cyclin B1,D1 mRNA和蛋白的高表达可促进食管鳞癌的发生与发展,在癌前病变期的鳞状上皮不典型增生组织中即出现高表达,是食管鳞癌发生过程中的早期事件.

【Abstract】AIM: To investigate the expressions of cyclin B1 or D1 mRNA and protein in esophageal squamous cell carcinoma (ESCC) tissue and their relationship to the development of ESCC. METHODS: Expressions of cyclin B1 and D1 mRNA and protein were respectively detected by in situ hybridization and immunohistochemistry SP in 62 case of ESCC(15 cases of grade Ⅰ, 25 cases of grade Ⅱ, 22 cases of grade Ⅲ; 20 cases with lymph node metastasis, 42 cases without lymph node metastasis; the depth of tumor infiltration: invasion of mucosa, submucosa, or shallow layer in 7 cases, of deep muscle in 14 cases, and of fiber membrane in 41 cases), in 31 cases of paracancerous atypical hyperplasia, and in 62 cases of normal esophageal mucosa. The correlations were analyzed between the expressions and the development of ESCC. RESULTS: Cyclin B1, cyclin D1 mRNA and protein were not expressed or low expressed in normal esophageal mucosa, but high expressed in paracancerous dysplasia and esophageal cancer tissues, and the difference among the 3 groups was statistically significant (P<0.05). Cyclin B1, D1 mRNA and protein expressions in ESCC tissues had nothing to do with the patients gender and age, but had a significant correlation with the histological grade, depth of invasion or lymph node metastases (P<0.05).
