2015年10月 第36卷第5期 牡丹江医学院学报 Journal of MuDanJiang Medical University October.20l5 Vo1.36 N0.5 2015 ・81・ 右旋美托咪啶是仅2受体激动剂,具有镇静、抗 焦虑、和抗交感神经作用,可以引起血压、心率的降 低。我们发现,在使用右旋美托咪啶10min后,血压 下降明显,与使用前有差异性,而心率虽下降,无差 异,当在使用芬太尼后,D组的血压、心率下降与麻 醉前相比具有显著差异,可能是芬太尼的协同作用。 因为芬太尼是常用的麻醉性镇痛药,临床使用时可 引起心动过缓。D组、N组在麻醉的气管插管的即 刻、插管后的1min、2min、3min各时点的血压、心率 明显低于麻醉前,而各时点之间组内及组间无显著 的差异,可能是气管插管时,应用了利多卡因喉头及 气管内的喷雾,使得气管插管后的血压、心率没有太 多的改变。有报道,利多卡因气管内表面麻醉可以 抑制气管插管的反应。 综合上述,麻醉诱导前预注右旋美托咪啶可以 降低交感神经的兴奋性,使患者安静,同时抑制芬太 尼引起的呛咳反应。 [2] [3] [4] [5] 参考文献 Tweed W A,Dakin D.Expiosive coughing after bolus fentanyl in— jection[J].Anesth Analg,2001,92(6):1442—1443.
