因此探讨甲亢患者脑损害的神经影像学特征有一定的临床价值和社会意义。
目的 通过静息态功能磁共振成像(rs-fMRI)分析甲亢患者脑自发活动异常模式,并探讨甲亢患者脑损害的神经影像学特征。
方法 选取2014年12月至2016年6月在新乡市中心医院门诊拟行131I 治疗的甲亢患者25例纳入甲亢组,同时选取医院和临近社区招募的性别、年龄、受教育程度与甲亢组无显著差异的健康志愿者28例作为对照组。
使用症状自评量表(SCL-90)评估研究对象的心理健康状况。
研究对象均接受rs-fMRI 检查,通过测量全脑比率低频振幅(fALFF)反映局部脑自发活动。
应用Pearson 相关分析探讨平均比率低频振幅(mfALFF)值与临床变量及心理健康状况之间的关系。
结果 甲亢组SCL-90焦虑、敌对评分高于对照组(P<0.05)。
与对照组相比,甲亢组右侧小脑蚓部7区〔蒙特利尔神经研究所(MNI)坐标:3、-75、-27,t=4.381〕、左侧丘脑(MNI 坐标:-6、 -6、0,t=3.545)、右侧丘脑(MNI 坐标:6、-3、3,t=4.356)的mfALFF 值升高,左侧小脑后叶(MNI 坐标: -39、-81、-27,t=-3.402)、左侧眶部额中回(MNI 坐标:-9、57、0,t=-5.253)、左侧额下回(MNI 坐标:-42、33、18,t=-3.747)的mfALFF 值降低。
相关性分析结果显示,右侧小脑蚓部7区的mfALFF 值与SCL-90焦虑(r=-0.419,P=0.037)、敌对(r=-0.402,P=0.046)评分呈负相关。

方法:回顾性研究联勤保障部队第九 四医院2015年6月至2021年6月收治的自身免疫性脑炎患者15例,并结合文献进行分析。
结果:15例患者中男性9例,女性6例。
10例为抗N 甲基 D 天冬氨酸受体(NMDAR)脑炎,2例为抗γ 氨基丁酸B型受体(GABABR)抗体相关脑炎,2例为抗富亮氨酸胶质瘤失活蛋白1(LGI1)抗体相关脑炎,1例为抗代谢型谷氨酸受体5(mGluR5)脑炎。
15例患者中有7例存在前驱感染样症状,5例起病前存在精神刺激因素;15例中出现精神行为异常13例,癫痫强直阵挛发作7例,不自主运动6例,认知障碍12例,意识障碍6例,自主神经功能障碍9例。
13例精神行为异常患者有12例症状缺乏典型性。
临床医生对自主神经功能障碍的识别率较低,9例中有7例被考虑为药物或感染所致。
15例患者中病重6例,病重的主要原因为意识障碍、中枢性通气不足、肺部感染。
15例患者中有10例头颅磁共振成像(MRI)存在新发异常病灶。
脑脊液检查示压力异常7例、蛋白异常6例、细胞数异常4例。
脑电图3例未做,余12例中有4例正常,8例呈慢波,病例6同时存在尖波。
结论:自身免疫性脑炎多伴精神症状,不典型的精神症状尤其伴早期应激心理因素者易被误诊为功能性精神障碍,且在治疗过程中,自主神经功能障碍的症状易被考虑为药物所致。

PalinkasDepartment of Family and Preventative Medicine, University of California, San Diego, La Jolla CAABSTRACTAnecdotal evidence of the individual and interpersonal problems that o ccurred during the Shuttle -Mir Space Program (SMSP) and other long-duration Russian/Soviet missions, and studies of personnel in other isolated and confined extreme (ICE) environments suggest that psychosocial elements of behavior and performance are likely to have a significant impact on the outcome of long-duration missions in space. This impact may range from individual decrements in performance, health and well being, to catastrophic mission failure. This paper reviews our current understanding of the psychosocial issues related to long duration space missions according to three different domains of behavior: the individual domain, the interpersonal domain and the organizational domain. Individual issues include: personality characteristics that predict successful performance, stress due to isolation and confinement and its effect on emotions and cognitive performance, adaptive and maladaptive coping styles and strategies, and requirements for the psychological support of astronauts and their families during the mission. Interpersonal issues include: impact of crew diversity and leadership styles on small group dynamics, adaptive and maladaptive features of ground-crew interactions, and processes of crew cohesion, tension and conflict. Organizational issues include: the influence of organizational culture and mission duration on individual and group performance, and managerial requirements for long duration missions. 方法便利选取2021年1—12月期间于福建省宁德市康复医院就诊的抑郁症伴焦虑症患者84例作为研究对象,以随机数字表划分为两组,即对照组(42例)与研究组(42例)。
对照组应用艾司西酞普兰治疗,研究组在此基础上联合舒肝解郁胶囊治疗。
针对两组临床疗效,治疗前与治疗8周时精神症状评分与神经递质水平,以及不良反应进行比较。
结果研究组总有效率为95.24%较对照组的78.57%高,差异有统计学意义(χ2= 5.126,P<0.05)。
治疗前各项精神症状评分比较,差异无统计学意义(P>0.05);研究组治疗8周时汉密尔顿抑郁量表(HAMD)及汉密尔顿焦虑量表(HAMA)分值低于对照组,差异有统计学意义(P<0.05)。
治疗前去甲肾上腺素(NE)、多巴胺(DA)、5-羟色胺(5-HT)比较,差异无统计学意义(P>0.05);治疗8周时研究组NE、DA、5-HT较对照组高,差异有统计学意义(P<0.05)。
研究组不良反应发生率16.67%与对照组11.90%比较,差异无统计学意义(χ2=0.389,P>0.05)。
结论在常规艾司西酞普兰的基础上联合舒肝解郁胶囊治疗抑郁症伴焦虑症效果可靠,可以有效改善患者抑郁与焦虑症状,以及神经递质水平,具有推广价值。

Methods From January to December 2021, 84 patients with depression with anxiety disorder who attended Ningde Rehabilitation Hospital during this pe‐riod were conveniently selected as research subjects and divided into two groups by random number table, namely, the control group (forty-two cases) and the study group (forty-two cases).
