3. Clinical manifestations
The younger the child is, the higher incidence of meningitis will be. ½-2/3 of cases occur less than 1yr of age.
Mode of presentation: • Acute or fulminant onset: symptoms and signs of sepsis; meningitis evolve rapidly over a few hours and death within 24 hours; usually infected with Neissria meningitides (N. meningitides).

Bacterial meningitis is an inflammation of the leptomenings, usually causing by bacterial infection. Bacterial meningitis may present acutely (symptoms evolving rapidly over 1-24 hours), subacutely (symptoms evolving over 1-7days), or chronically (symptoms evolving over more than 1 week).

食、禁水，术前抗 生素皮试、带药、更换清洁病员服、剃头，留置 导尿。与手术室人员进行患者、药物核对后，送 入手术室。
• 术后护理 • ①常规观察生命体征、意识状态、瞳孔、肢体活 动状况等。颅前窝手术后常有额眶部水肿，可给 予冷敷以减轻不适。病人取半卧位、抬高头部以 减轻脑水肿。
护理措施
• 术前护理 • ①心理护理：给予适当心理支持， • ②饮食护理：给予易消化、高纤维、高蛋白、高 热量饮食。必要时给予静脉输入高营养液，以改 善患者的全身营养状况，增强机体抗病能力。 • ③病情观察及护理：注意观察患者神志、瞳孔、 生命体征变化。观察颅内压增高的征象，如患者 头痛加剧，呕吐频繁，反应迟钝，意识障碍加深 ，此时应警惕脑疝的发生。遵医嘱按时按量给予 抗生素。
• ②疼痛护理 切口疼痛多发生于术后24小时内，给 予一般止痛剂可奏效。颅内压增高所引起的头痛 ，多发生在术后2～4日脑水肿高峰期，需依赖脱 水、激素治疗降低颅内压。
• ③营养和补液 一般颅脑手术后1日可进流质饮食 ，第2、3日给半流饮食，以后逐渐过渡到普通饮 食
• ④脓腔引流管护理 保持通畅，勿折叠、扭曲、压 迫管道。妥善固定引流管，引流瓶(袋)应至少低 于脓腔30cm，患者应取利于引流的体位。

脑脓肿(Brain abscess)是各种化脓性细菌 侵入脑实质内所形成的脓肿，它可使脑组织遭 受到直接的破坏，是一种严重的颅内局限化脓 性疾病。
症状、体征
1、全身性感染症状：发热、头痛、全身乏力、肌肉酸 痛等表现。 2、颅内压增高症状：持续性头痛，呕吐、缓脉、呼吸 变慢、嗜睡、意识模糊。 3、局灶性症状：额叶脑脓肿常出现表情淡漠、记忆力 减退、局灶性或全身性癫痫发作、对侧肢体瘫痪和运 动性失语等；颞叶脓肿常有欣快、健忘、感觉性失语 或命名性失语、偏盲及轻偏瘫等；小脑脓肿则可出现 强迫头位、肢体共济失调等。 4、脑疝形成和脓肿破溃。

• 脓肿的复发问题：（1） 原发感染病灶未处理或未彻底处 理，继续颅内侵入。
（2）手术时脓肿破溃、脓液外渗、污染创口，日后形成新的 脓肿。
• 有认为当全身大剂量应用抗生素时药物能进入脓肿腔，所 以没有必要在脓肿腔内直接滴注抗生素。
• 对脓肿而言，抗生素治疗的时间很大程度上取决于外科引 流的效果和感染源对抗生素的敏感性。在大多数病例推荐 6~8周的抗生素治疗，脓肿的消退依靠连续的影像学检查、 监测。脓肿的完全消退和对比增强异常的消失可能需要 3~4月。对比增强的残存区可能存在6个月，并且在一小部 分病人这些脓肿可能复发。

• 影响患者预后的指标还是病人在就医时的神经系统状态。
• 若治疗正确，多发脑脓肿的生存率与单发脑脓肿相似。
• 癫痫和脑积水是常见的后遗症，在急性期，大约10%病例 发生癫痫，随后，常在诊断后的数年开始，35~75%发生癫 痫。
• 所有幸存者中伴长期神经系统后遗症者发生率30~50%

The cases are usually elder or pediatric male patients. The incidence of BA is 8% of intracranial masses in developing countries and 1-2% in the western countries. Mortality from a BA has recently decreased from about 50% to 20%, mostly as a result of introduction of CT scanning that resulted in earlier diagnosis and accurate localization.

Epidemiology
BA is more common among men –twice to three times. Morbidity rate is highest in fourth decade of the life. BA still continues to be a significant problem in the developing world due to large scale poverty, illiteracy, and lack of hygiene.
Mortality is mainly influenced by : Age Neurological condition at admission; Delays in hospitalization, Focal neurologic deficits at admission, Impaired host immunity, Uncontrolled diabetes mellitus, Glasgow Coma Scale (GCS) <12 associated with death and permanent neurologic deficits.
