烧伤患者的麻醉第一节烧伤的基础理论知识一、烧伤的临床过程烧伤患者部分或全部丧失了皮肤保持机体内环境稳定的功能，引起一系列复杂的病理生理变化，通常将烧伤临床过程分为四期：体液渗出期、急性感染期、创面修复期和康复期，各期之间相互交错，联系密切。
（一）体液渗出期1.毛细血管通透性增加，组织水肿。
2.皮肤屏障作用丧失，大量水分经皮肤蒸发。
3.伤后立即开始，体液渗出的量与烧伤严重程度呈正比，6～12小时内最快，24～36小时逐渐减小至停止，严重烧伤可能延续至48小时以上。
（二）急性感染期1.伤后1～2周内出现。
2.感染主要滋生地为创面，甚至出现全身脓毒血症。
3.应尽早处理创面、消除病灶、防治休克，提高机体抵抗力。
（三）创面修复期1.创面肉芽组织的出现，机体形成初步的防御线，细菌自创面进入的机会减少。
2.加强营养，增加机体抵抗力，积极消灭创面，防治感染。
（四）康复期积极功能锻炼，常需多次整形修复。
二、烧伤面积的估计目前应用较多的是中国九分法。
头部体表面积：9%（1个9%）双上肢体表面积：18%（2个9%）躯干含会阴：27%（3个9%）双下肢含臀部：46%（5个9%+1）合计：11×9+1=100%三、烧伤深度的估计1.Ⅰ°：红斑性烧伤，伤及表皮层，增生活跃，可3～5天内自愈不留瘢痕。
2.Ⅱ°浅Ⅱ°：水疱性烧伤，伤及表皮层及部分真皮乳头层，如无感染不留瘢痕。
深Ⅱ°：伤及真皮乳头层以下，临床变异多。
无感染，也可自愈，合并感染需手术植皮。
3.Ⅲ°：焦痂性烧伤。
全层皮肤烧伤甚至深及脂肪、肌肉等。
四、烧伤严重程度分级1.轻度总面积9%以下的Ⅱ°烧伤2.中度总面积10%～29%或10%以下Ⅲ°烧伤3.重度总面积30%～49%或10%～19%Ⅲ°烧伤；或烧伤面积不足30%，但有下列情况之一者：①全身情况较重或已有休克；②复合伤；③中、重度吸入性损伤。
