烟台市残疾儿童精准康复救助申请审核表
县级：年度：救助册编号：（□全日制□非全日制）
填表说明：
一、此表只用于残疾儿童进市定点康复机构接受康复训练及服务用表，家长
（监护人）可在市定残疾儿童康复机构领取，将此表个人（监护人）信息部分填写完整签字确认，康复机构出具接受意见，即同意或不同意，并签字盖章。
二、定点康复机构需在本周期到机构康复时间一栏处，标明残疾儿童到本机构康复之日（即为训练开始之日，均在县级残联审核日期之后或同日，需与康复训练档案日期一致，）。
三、残疾儿童家长持本表到县级残联处审核，本表一式两份，由县级残联留存1 份，由家长带回定点康复机构存入康复档案1 份。
四、“康复需求项目”栏，必须选择与残疾证类别相符的项目。
五、0-6 岁无证的残疾儿童选择项目需和烟台市级二甲以上医院出具的疑似残疾诊断证明相符。
六、县级残联审核意见一栏，需填写审核人姓名，并加盖残联公章，签字盖章即默认同意，无需填写其他内容。

二、受助儿童监护人职责和义务
1.不随意更换定点机构或中断康复训练。
2.按项目规定时间（次数）接送受助儿童。
3.配合机构开展家庭康复训练，提高康复质量。

疑似病例先登记，待确诊残疾后再录入。
附表1-1 山东省0-6岁残疾儿童康复需求调查统计汇总表（一）33注：1、此表以乡、镇（街道）为单位汇总，根据《山东省0-6岁残疾儿童康复需求调查表》（附表1）有关指标统计后报县（市、区）调查办汇总。
2、调查办根据疑似患儿汇总情况，组织医疗机构临床医生，采取集中与入户相结合的方式逐人核查、明确诊断后，修改“确认”栏数据，清空“疑似”栏数据，将此表汇总后，于2012年5月31日前上报市调查办。
3、根据实际情况在对应栏目内打“√”，没有的则空格。

根据《山东省0-6岁残疾儿童康复需求调查统计汇总表（一）》完成此表汇总，于2012年6月10日前将此表电子版报省残联康复部。
附表2山东省聋儿康复训练机构调查表市县（市、区）（盖章）填表人电话填报时间：2012年月日55注：1、此表由各县（市、区）残联组织调查，于2012年5月31日前报市残联康复部汇总。
各市汇总后于6月10日前将正式件（盖章）报省残联康复部，同时报电子版。
2、此次调查结果要做为年度报表统计依据，市、县各级要立档存查。
3、“兼训其他类别儿童”：指本机构同时开展脑瘫、智障、孤独症康复训练，并有相对独立的业务管理，在对应栏内注明类别名称，同时填写对应类别的调查表。
“省儿童救助定点机构”指承担国家和省残疾儿童康复训练任务的定点机构。
