晨间护理操作程序
目的：
（1）使病人清洁、舒适，预防压疮及肺炎等并发症。
（2）观察和了解病情，满足其身心需要，促进护患沟通。
（3）保持病床和病室整洁。
评估：
（1）病人的病情、年龄、意识状态、自理能力、文化背景、生活习惯及睡眠状况。
（2）病人皮肤的受压状况及各种治疗性导管、牵引等固定维持情况。
（3）病人床单位是否清洁、平整。
（4）病人的心理状况。
计划：
（1）用物准备：一般病人自备漱口用具、毛巾、面盆、梳子、肥皂、50％乙醇。
重症病人另备口腔护理盘、便盆等；备清洁衣裤、清洁床上用品、床刷、扫床巾。
（2）环境准备：酌情关闭门窗，调节室温。
（3）病人准备：根据病情身体状况取舒适卧位。
（4）护士准备：着装整齐，戴口罩，洗手，备齐用物。
实施：
（1）操作步骤：
护士携物至床旁→向病人解释→放平床支架→协助排便→协助漱口（口腔护理）、洗脸、洗手、梳头→翻身，检查皮肤受压情况→擦洗背部并按摩→整理床铺（必要时更换被服等）→整理床单位→开窗通风→整理用物。
（2）注意事项：

②对能自理的病人协助做好清洁工作。

用物准备：护理车上层：护理篮、口腔护理用物、香皂 95%酒精、大毛巾1条、小毛巾1条、衣裤
1套、大单、被套、水温计
下层：便器及盖便器布、尿布、热水桶（内
盛47-500C热水）、污水桶、面盆各1、镊子、
一次性手套
环境准备：室温适宜，关门窗或屏风遮挡，松盖被，按
需给便器（步骤：脱裤→放胶单→放便盆→
撤便盆→洗手）
口腔护理协助患者取舒适体位，按口腔护理流程
倒水入盆，铺大毛巾在被头
擦洗洗脸：眼（内眦→外眦）→额→鼻→两侧颊→耳后→
下颌→颈
松裤协助病人侧卧，面向操作者
洗手（床边铺大毛巾）→换水
擦背：脱衣（酌情而定，如不脱，把衣服向颈部卷）
→铺大毛巾→分3条线擦洗，擦2遍（第1、3
条线从肩至臀部，注意擦腋后线，第2条线从
颈至骶尾部）
视病情需要按摩按摩骨隆突部（从骶尾部向上按到第七颈椎，单手按摩）
→更换或整理衣服
梳头协助患者取舒适体位→梳头（垫巾在枕头上）→剪指
甲、趾甲（剪指甲时护士背向病人，避免指甲弹向病人）
