ABSTRACT:Objective To investigate the therapeutic effect of intensive glycemic control on patients with early diabetic nephropathy.Methods A total of 41 type 2 diabetes patients who developed microalbuminuria were divided into two groups randomly.Patients in Group A received intensive glycemic control and the blood glucose in Group B was regularly controlled.Glycemic monitoring and contro1 were foflowed for 12 weeks to observe the changes of microalbuminuria in both groups;meanwhile the levels of serum lipids and coagulation indices were also recorded.Results The urine albumin excretion rate(UAER)in Group A decreased significantly from(47.91±13.86)mg/24 h to(35.31±14.56)mg/24 h after 12 weeks(P<O.05),and this decrease was significantly greater than that in Group B.However,Group B had no significant difference in UAER decrease[(48.93 4-13.32)mg/24 h to(4O.48±19.62)mg/24 h,P>0.05].The decrease of triglyceride(TG)and low-density lipoprotein cholesterol(LDL cholestero1),and the increase of high-density[ipoprotein cholesteroI(HDL cholestero1)showed no significant differences(P>0.05).And the Ievel of plasma fibrinogen(FIB)showed no significant decrease after 12 weeks,either(P>0.05).Oonclusion Intensive glycemic control reduces the IeveI of microalbuminuria and may ameliorate the progression of early diabetic nephropathy. was designed to investigate the therapeutic effect of intensive glycemic control on type 2 diabetes pa- tients with early DN. 2 Subjects and methods 2.1 Patients The study was performed at the Outpatient Di- abetic Clinic of the First Affiliated Hospita1.Medi. cal School of Xi an Jiaotong University.A total of 4 1 type 2 diabetes patients with microalbuminuria were selected.Informed consent was obtained from all the patients and the risks and benefits of the stud— Y were explained to them.The patients had su ffered from diabetes for at least two years and were admin- istered with hypoglycemic agents and/or underwent diet contro1.They were diagnosed as early DN ac- cording to the 1 9 9 9 WHo criteria[ ]and Mogenson classi cation[6。. Microalbuminuria was defined as urinary albumin excretion rate(UAER)of 20 to 200 mg/min or 24 hour urinary albumin of 30 to 300 mg/24 h.In this study,UAER was assayed on a 24一hour urine sample.The 24-hour urine samples of each patient were collected twice for UAER assay to confirm the diagnosis.The following diseases that might af ̄ct the UAER results were excluded,such as glomerular nephritis,nephrotic syndrome,urina— ry tract infections,chronic diarrhea,heart failure, tuberculosis and recent medication of nephrotoxic drugs.Those who were accompanied with other se- vere diabetic complications or severe diseases o f oth- er systems were also excluded.Blood pressure was under control with a target of 1ess than 1 30/80 mmHg in hypertensive adults with diabetes.Angio- tensin converting enzyme inhibitors(ACEI)and an-
