The ways to treat liver ascitesThe ways to treat liver ascites As the morden medicine's development，west doctors still have two shortcomings to treat liver ascites .However，this two shortcoming are west doctor's main ways and strong suits to treat liver ascites .Now,I will claim that I am not repel or defame west doctor,this is a fact.Consider the following,the reasons of west doctor's shortcoming :For example,putting on a drip.As we all know,when someone have a cold and puting o a drip,he or she usually go to WC repeatedly. For this reason,as liver ascites's pantients they have a little excretory functions,even they have no excretory functions in one day .According to this ,putting on a drip is not a good way to treat liver ascites ,but it will make the pantient's liver become worese and worese.Drawing water by centesis:The reason of liver ascites is that when liver suffer from viruses that the'Y ang Qi' will be redused.For this reason ,the water will leave in the body.When the liver ascites comes to metakinesis or later period,west doctcrs only use drawing water by centesis to reduse the water which in the body.However,this way only can reduce the water in a short time,and can not make the patient recure.Maybe it will make the liver ascites becomes worse.Becouse there are some energy that we called "Y ang Qi".It can prevet the redundant water stopping in the body.When your liver suffer from viruses ,your "Y ang Qi" wil reduce.Then the redundant water will leave in your body.Such as edema nephrotic ,thoracic effusion,liver ascites and so on.The "Yang Qi"is seemed to be the sun.As we all know,when it is sunny that the wet thing will become dry quickly.In this time ,I use the analogy only want to tell you that when somebody's "Y ang Qi"is adequate enough,he will not have redundant water in his body.On the other hand,I will explain that drawing water by centesis is not a good way to treat liver ascites,but cause the "Y ang Qi"being lost.Once 'Y ang Qi'is lost,version's liver will lose the function that can prevent redundant water in our body.It also explain why drawing water by centesis can cause liver ascites become worese and worese.The above is my comment to west doctors' way to treat liver ascites .Excuse me that I can't have an adequate account to common people.The comment to west doctors' way ,I only can stop here.Because I don't want someone to misunderstand me that I or the east doctors repel or defame west doctors.The east doctors treat liver ascites is based on the age-old history.The ancinet doctors found that liver ascites are caused by bad emotion, and other undefined ways.When the liver ascites comes to metakinesis or later period,the 'Y ang Qi 'will reduce ,and the redundant water will leave in the body.So I think the good way to treat liver ascites should be based on "Removing liver,promoting blood circulation to remove blood stasis ,replenishingqi to invigorate the spleen,diureses and be in harmonious Ying and Y ang ' ,this is the good and comprehensive's way to treat liver ascites .肝腹水的治疗论在目前医学的发展突飞猛进的年代，西医目前在治疗肝腹水的方面，仍存在两种重大失误，而这两大失误恰恰是西医用于治疗肝腹水的特长与办法。

但我要声明的是，这不是本人更不是我国中医针对西医的排斥和诋毁，而是事实。

西医的两大失误原因如下：（一）如打“点滴”来说，例如：一个正常人伤风感冒时，在打点滴治疗过程中，常常出现小便繁多，这一点是人所共知的，因此对一个肝腹水患者来说，肝腹水患者原本就很少大小便、甚至有一二日都不解一次大小便的情况出现：凭此这一点却说明了的是，点滴不但对肝腹水病情起不道好的疗效，反到会使肝腹水的病情逐进一步更快的恶化道理。

（二）穿刺抽水的疗法：肝腹水的形成多为肝脏受病后，所引起体内的阳气衰微，才导致了水湿内停的结果。

当肝腹水病情发展至中、晚期的时期，西医治疗的唯一方法，就是用穿刺抽水的方法来消除腹水的膨胀。

而穿刺抽水法却只是用来缓一下腹水膨胀的一时之急，其根本就无法治愈肝腹水病情的本质，反则还会逐使病情发展恶化的更快。

原因是人体内有一种医学称之爲〈阳气〉的热量，而这种热量却就是人体脏腑抵御水湿产生内停的一种特有的重要功能，这种热量也就是医学所称之的阳气。

当人体脏腑受病时而导致体内阳气衰微，因此引发水湿内停的结果，如肾病水肿、胸积液、肝腹水等等。

例如，人体内的阳气好比外界大自然的太阳一样，世人皆知阳光好的天气，水湿物体不但干的快而更干彻底：在此本人借用这样的比喻只是想说明的是，人的体内阳气旺盛，才不会发生水湿内停形成水肿的道理。

同时也是借此来说明穿刺抽水的方法不但治愈不了肝腹水病根本，反而还会导致人体内阳气换散外遗，一但阳气换散外遗，这就意味着的是人体脏腑失去了抵御水湿内停的特有功能，也说明了的是穿刺抽水的疗法会导致腹水病情恶化更快的道理。

以上是本人简单的对西医在治疗肝腹水方面的评论，同时也是对肝腹水患者及家属们的提醒。

非常抱歉，恕我不能对世人更详细的说明，对西医在这方面的评论，因爲我考虑到，爲了避免有人认爲是我，或我国中医有意对西医的针对性排斥与诋毁，所以我只能讲到这里，敬请谅解！中医对于在治疗肝硬化腹水的方面，是根据几千年以来，我国历代医家在不断坚懈不怠的探讨脏腑功及其病理变化时，发现肝硬化腹水的形成多由情志不舒、或大怒伤肝等等，同时也不排除有其它不明的原因，引起肝气郁结，肝失条达疏泄而导致经络堵塞不通引起肝硬变的形成。

当肝硬变的形成到了中晚期时，随之而引起人体阳气的不足，使水湿发生障碍内存。

因此，在治疗肝硬化腹水之根本时，本人认为应以“疏肝通络、活血软坚、化气利尿、温阳益气”为主，此方为针对肝硬化腹水全面性攻补兼施的正觉治疗之法。

方法　对照组家兔未注射氢氧化铁；氢氧化铁设计２个不同浓度，３个不同剂量，得到６个实验组，实验组家兔麻醉、插管、注入氢氧化铁后，观察家兔口唇皮肤颜色、呼吸频率（ＲＦ）、心率（ＨＲ）、血压（ＢＰ）、中心静脉压（ＣＶＰ）、右室收缩压（ＲＶＳＰ）、右心功能指标［＋ｄｐ／ｄｔｍａｘ（右室压力上升最大速率）、右室舒张压（ＲＶＤＰ）、－ｄｐ／ｄｔｍａｘ（右室压力下降最大速率）］等栓塞前后变化情况，紧接着快速输入生理盐水后，观察家兔口唇皮肤颜色、ＲＦ、ＨＲ、ＢＰ、ＣＶＰ、右心功能指标等变化情况，复制家兔急性右心衰竭模型。

结果　各实验组家兔均发生不同程度的急性右心衰竭，栓塞成功，与对照组比较，各实验组家兔口唇紫绀、皮肤黏膜苍白；ＣＶＰ、ＲＶＤＰ、－ｄｐ／ｄｔｍａｘ显著增加（Ｐ＜０．０５），ＢＰ、ＲＶＳＰ、＋ｄｐ／ｄｔｍａｘ显著降低（Ｐ＜０．０５）；分别与实验组５和实验组６相比，对照组、实验组１、实验组２、实验组３家兔ＲＶＳＰ、＋ｄｐ／ｄｔｍａｘ显著增加（Ｐ＜０．０５）。

栓塞成功并注入１０ｍＬ、３０ｍＬ、５０ｍＬ、７０ｍＬ生理盐水后，分别与实验组５和实验组６相比，对照组、实验组１、实验组２、实验组３家兔ＣＶＰ显著下降（Ｐ＜０．０５）；此外，栓塞成功并注入９０ｍＬ生理盐水后，分别与实验组５和实验组６相比，对照组、实验组１、实验组２、实验组３家兔ＣＶＰ、ＲＶＤＰ、－ｄｐ／ｄｔｍａｘ均显著下降（Ｐ＜０．０５）；与其他组家兔相比，实验组５和实验组６家兔胸水、腹水、肝腹水程度更严重，心腔体积更大。

在临床实践中经常遇到腹水患者，其病因复杂多样，涉及多系统和多种疾病。

肝硬化门脉高压症是腹水形成的主要病因之一，占所有腹水成因的75%，其余原因包括恶性肿瘤（10%）、心力衰竭（3%）、结核（2%）、胰腺炎（1%）或其他少见的原因[1]。

本文简要介绍腹水的病因，为临床诊治提供线索。

1 肝源性腹水1.1 肝硬化性腹水肝硬化是由不同病因长期作用于肝脏引起的慢性、进行性、弥漫性肝病，常见病因有病毒性肝炎、慢性酒精性肝病、非酒精性脂肪性肝病、自身免疫性肝病等。

肝硬化会导致门脉高压，而腹水是肝硬化门脉高压症最常见的并发症之一[2-5]，约50%代偿期肝硬化患者10年内会发生腹水[6，7]。

腹水不但影响患者的生活质量，而且增加了自发性细菌性腹膜炎、上消化道出血、肾功能衰竭或肝性脑病等风险，是预后不良的征象。

肝硬化并发腹水患者1 a和 5 a病死率分别为 15%和 44%[3，8]。

肝硬化腹水的形成机制复杂，尚未完全阐明。

主要的观点有以下几种学说：（1）充盈不足假说：门静脉高压症和腹水形成在前，循环功能障碍和肾脏损伤在后[9，10]。

门静脉高压和低白蛋白血症导致内脏淋巴形成增加。

过多的淋巴超过了胸导管的引流能力，使液体外渗入腹腔，从而影响循环和肾功能，导致血容量和心输出量减少，增加外周血管阻力；（2）周围动脉扩张假说：这一假设主要是说循环功能受损，其特征是肝硬化门静脉高压引起内脏血管扩张和动脉有效血容量下降。

因为血管舒张因子的产生和活性增加，包括一氧化氮、一氧化碳和内源性大麻素等[11-13]，导致全身血管阻力（systemic vascular resistance，SVR）下降[14]；有效循环血量的相对不足刺激压力感受器，引起肾素-血管紧张素-醛固酮系统和交感神经系统的活化以及抗利尿激素的分泌，导致肾脏对钠水重吸收增加[15]，进一步加重腹水。
