本章主要介绍物理治疗-运动疗法。
一、概述1、运动疗法(therapeutic exercise)指以生物力学和神经发育学为基础,采用主动和被动运动,通过改善、代偿和替代的途径,旨在改善运动组织(肌肉、骨骼、关节、韧带等)的血液循环和代谢,促进神经肌肉功能,提高肌力、耐力、心肺功能和平衡功能,减轻异常压力或施加必要的治疗压力,纠正躯体畸形和功能障碍。
2、基本原则(1)因人而宜按照各个患者功能障碍的特点、疾病情况、康复需求等制定康复治疗目标和方案,并根据治疗进度和功能及时调整方案。
(2)循序渐进应激适应性要逐步建立,训练效应符合量变到质变的积累过程,参加康复训练是技能学习过程,神经-肌肉功能重建也是系统再学习的过程,因此运动强度应该由小到大,运动时间由短到长,动作复杂性由易到难,休息次数和时间由多到少、由长到短,训练的重复次数由少到多,运作组合由简到繁。
(3)持之以恒训练需要持续一定的时间才能获得显著效应,停止训练后训练效应将逐步消退。
因此康复训练需要长期持续,甚至维持终生。
(4)主动参与强调患者主动参与康复训练。
只有主动参与,才能获得最佳的治疗效果。
运动功能不可能通过被动治疗而得到最大限度的恢复。
(5)全面锻炼人体的功能障碍是多器官、多组织、多系统功能障碍的综合,康复的目标应包括心理、职业、教育、娱乐等多方面,最终目标是重返社会。
因此康复治疗应该全面审视,全面锻炼。
