Submission of Application FormApplicants should provide true, accurate and complete information in support of their applications. Any misrepresentation will result in disqualification of their applications and subsequent enrolments in the University.(i) For all applications <except Hong Kong PhD Fellowship Scheme (HKPFS)>a.Read carefully these Notes for Applicants.b.Choose “Apply Now”. Select the Faculty and then the programme apply for and press the “ApplicationForm” button at the bottom.plete the application form and press the “Submit” button. Y ou will see an acknowledgementreceipt if the submission is accepted. Print it for future reference and mark down the Application No.printed on the top right hand corner. If you see any error message(s), please enter the requiredinformation according to the instructions.d.Download forms for supporting documents. All applicants MUS T print 2 “ConfidentialRecommendation” and “Transcript Submission Covering Sheet” for further actions. Print “PaymentV oucher” only if you choose to pay by bank draft/cheque.e.Submit a separate application form along with the supporting documents for each programme.(ii) For applications of Hong Kong PhD Fellowship Scheme (HKPFS)a.Go to RGC website .hk/hkpfs/apply.html to submit an initial application and get aHKPFS reference number.b.Read carefully these Notes for Applicants.plete the application form and press the “Submit” button. Y ou will see an acknowledgementreceipt if the submission is accepted. Print it for future reference and mark down the Application No.printed on the top right hand corner. If you see any error message(s), please enter the requiredinformation according to the instructions.d.Download forms for supporting documents. All applicants M UST print 2 “Referee’s Report” and“Transcript Submission Covering Sheet” for further actions. Print “Payment V oucher” only if youchoose to pay by bank draft/cheque.e.Submit a separate application form along with the supporting documents for each programme.3. Submission of Supporting DocumentsAll applicants who apply for admission in fall 2013 should submit the following supporting documents to the relevant Divisions by the deadline and upload softcopies to the related website (https://.hk/onlineapp/upload_login.aspx). Otherwise, their applications may not be further processed. Addresses of Divisions are given in the Postgraduate Prospectus. All documents submitted will not be returned and will be destroyed if the application is unsuccessful. Documents which are not in English or Chinese should be accompanied by an official certified translation in either English or Chinese. Please always quote the Application No. in your future correspondences.a.Copies of certificates of academic/professional qualifications (e.g., university certificate of graduation,examination certificates and other qualifications).b.Official transcripts (with a complete record of courses attended and examination results) of all tertiarylevel studies.(i) Non-CUHK qualifications: Photocopies will not be accepted. Applicants should first downloadCovering Sheet” and send it to their universities to apply for transcripts. Transcripts should be returned directly to relevant Divisions in sealed envelope.Certificates and transcripts, which are not in English or Chinese, should be accompanied by anofficial certified translation in either English or Chinese.(ii) CUHK qualifications (Bachelor’s or higher degrees): Photocopies of transcripts issued by CUHK are accepted, but Academic Reports will NOT be accepted. Applicants should firstdownload the “Transcript Submission Covering Sheet” and attach it to the transcript copy.Please return the transcript copy to the relevant Divisions directly. CUHK students with otherqualifications not awarded by CUHK are required to follow the requirement listed in b(i). TheUniversity reserves the right to require applicants to submit the original transcript if deemednecessary.c.Applicants with degrees awarded by overseas universities by distance learning or by completing acurriculum of short duration may be required to provide assessment report from the Hong KongCouncil for Accreditation of Academic & Vocational Qualifications (HKCAAVQ) on the level ofqualification obtained. If necessary, applicants will be notified by Divisions concerned / the GraduateSchool separately.d.Confidential recommendations (for all applicants <except HKPFS>) or Referee’s report (for HKPFSapplicants only) from 2 referees (not from the applicant). Please download the “ConfidentialRecommendation” (see Sec tion 2(i)(d) above) or “Referee’s Report” (see Section 2(ii)(d) above) andsend one to each of your referees and request them to return the recommendations in confidence to therelevant Division as soon as possible or not later than two weeks after the application deadline of theprogramme.e.One photocopy of any one of the following score reports/certificates, showing that you have fulfilledthe English Language Proficiency Requirement for admission to the Graduate School:(i) a pass in English of HKALE, or HKHLE, or CUHK Matriculation Exam.; or(ii) a degree from a university in Hong Kong or an English speaking country; or(iii) valid official score report of TOEFL, or IELTS (Academic), or GMA T (verbal), or College English Test (CET6) of PRC, or Public English Test System (PETS-5) of PRC (with pass gradesin both written and oral parts) <HKPFS only accepts score report of TOEFL or IELTS(Academic)>; or(iv) a recognized professional qualification awarded in Hong Kong or an English speaking country.f. One photocopy of your HKID card or other identity document as given in the Application Form.g. Specific materials required by the relevant Division as listed in the Postgraduate Prospectus.h. The University may require applicants to submit additional information or documentary proof ifdeemed necessary.4. Application FeeThe application fee for admission to one postgraduate programme is HK$180/US$24. The application fee, once paid, is not refundable. Applicants may choose to pay by credit card or bank draft/cheque. If you pay by bank draft/cheque, please download the “Payment V oucher” and send it to the Division together with the payment.5. Selection ProcessDivisions will begin the selection process upon receipt of the applications and supporting documents.Individual programmes may require applicants to take an entrance test or attend interviews. Applicants will be notified by Divisions concerned separately.6. Application Resulta. For applicants who apply for PhD programmes through HKPFS, Research Grants Council will announceresults by late March 2013.b. For applicants who apply for MPhil/PhD or taught programmes directly with the University (i.e.non-HKPFS applicants), application result will be posted on the homepage of the Graduate School (.hk/gss) from mid-February to August 2013. Successful applicants will receive the offer letter by August 2013. Applicants will not be informed separately if their applications are not successful.7. RemarksThe information collected from the application form will be used for the following purposes:a. as a basis for selection of applicants for admission in 2013-14 to postgraduate programmes offered by theUniversity and other related purposes;b. for verifying information about the applicant’s publ ic examination results and academic record in relevantinstitutions in Hong Kong and elsewhere;c. for identifying records of previous studies as a student in the University or other institutions;d. as part of the applicant’s student record upon registrat ion for a programme, which will be used for allpurposes relating to his/her studies in accordance with the procedures of the University; ande. if necessary, maybe be transferred to other units within the University to facilitate verifications,communication, operations and planning.8.
