考核管理
➢由医务科组织成立院内业务考核专家小组，并组建题库，负责对 临床医师进行业务水平测试。 ➢每项考核工作结束后，由医务科通过院周会/科主任会形式对考核 结果进行公示。 ➢医务科负责将考核结果如实录入医师个人医疗档案，作为职称晋 升、岗位聘用的依据。 ➢医院把“三基三严”训练工作纳入基础医疗质量管理，与科室质 控挂钩；对“三基三严”训练工作做得好的给予表彰，对成效突出 的科室和个人给予奖励；对不重视“三基三严”训练工作，教育培 训管理混乱，缺乏规范的原始记录，“三基三严”考试考核不合格 者，给予通报批评、限期整改，根据情节轻重，扣罚科室质控分及 个人绩效工资。

喉乳头状瘤 临床表现
症状：
进行性声嘶，甚至失声 可喉喘鸣，重者呼吸 困难 儿童易喉梗阻 （喉腔较小，生长快， 多发）
喉乳头状瘤 临床表现
喉镜：
肿瘤呈乳头状、 淡红或暗红； 成人单发，小儿 多发广基 声门区多见，也 可发于声门上及 声门下
喉乳头状瘤治疗
1、手术：
显微支撑喉镜应用CO2激光切除肿瘤， 喉裂开手术逐渐淘汰 气管切开术（缓解呼吸困难）
4.垂直半喉切除术： 适用于一侧声门型喉癌。
5. 喉次全或喉近全切除术：包括环状软骨上喉部分切除术 （SCPL）,喉水平垂直部分切除（3/4喉切除），以及 Tucker喉次全切除和pearson喉近全切除术。
6、全喉切除术：
适用于中晚期不宜 进行部分喉切除术 的病人
食管发音法:将气体吞入食道，再经食管冲出， 产生声音，经咽腔和口腔调节，构成语言。

• 概念：良性中最常见， 10岁以下多见，极易复 发。成人癌变可能；
• 病因： HPV病毒，HPV-6，11。 母亲尖锐湿疣与儿童乳 头瘤的关系
• 病理：复层鳞状上皮， 富含血管，一般不浸润 基底膜
有浅溃疡、多有肺结核。活检可证实。
喉癌鉴别诊断 喉梅毒：声嘶粗而有力，喉痛较
轻，喉前部，粘膜红肿，有深溃疡， 血清反应阳性。
喉癌鉴别诊断
喉乳头状瘤：儿童好发，病程长，丛状、多发 性生长，声门运动好，活检确诊
喉癌治疗
治疗手段包括：手术、放疗、化疗及生物治疗；
治疗策略：以手术为主，放疗及其它疗 法为辅的综合治疗。
