T细胞膜表面有100多种特异性抗原,现已制备了多种单克隆抗体,WHO(1986)统称为白细胞分化抗原(cluster differentiation,CD)。
例如CD3代表总T细胞,CD4
代表T辅助细胞(TH),CD8代表T抑制细胞(TS)等。
应用这些细胞的单克隆抗体与T细胞表面抗原结合后,再与荧光标记二抗(兔或羊抗鼠IgG)反应,在荧光显微
镜下或流式细胞仪中计数CD的百分率。

【临床意义】
①CD3降低:见于自身免疫性疾病,如SLE、类风湿关节炎等。
②CD4降低:见于恶性肿瘤、遗传性免疫缺陷症、艾滋病、应用免疫抑制剂者。
③CD8减低:见于自身免疫性疾病或变态反应性疾病。
④CD4/CD8比值增高:见于恶性肿瘤、自身免疫性疾病、病毒性感染、变态反应等;CD4/CD8比值减低:见于艾滋病(常<0.5)。
⑤监测器官移植排斥反应时CD4/CD8比值增高预示可能发生排斥反应。
⑥CD3、CD4、CD8较高且有CD1、CD2、CD5、CD7增高则可能为T细胞型急性淋巴细胞白血病。

白细胞分化抗原还广泛分布于非造血细胞如血管内皮细胞、成纤维细胞、上皮细胞、神经内分泌细胞等。
白细胞分化抗原大都是跨膜的蛋白或糖蛋白,含胞膜外区、跨膜区和胞浆区;有些白细胞分化抗原是以糖基磷脂酰肌醇连接方式,锚定在细胞膜上。

在世界卫生组织(WHO)和国际免疫学会联合会(IUIS)的组织下,自1982年至1993年先后举行了五次有关白细胞分化抗原的国际学术讨论会。
并应用以单克隆抗体鉴定为主的聚类分析法,将识别同一分化抗原的来自不同实验室的单克隆抗体归为一个分化群(cluster of differentiation,CD)。

CD在临床免疫学研究中主要用于:①机体免疫功能的检测;②白血病、淋巴瘤免疫分型;③免疫毒素用于肿瘤治疗、骨髓移植以及移植排斥反应的防治;④体内免疫调节治疗。
在许多场合下,抗体及其识别的相应抗原都用同一个CD序号,因此在参阅教科书和文献时需加注意:(1)CD是流水编号,CD129暂缺;CD67和CD66b是重复的;CD187-190,CD211,CD237,CD341-343,CD345-248暂未编号。
(2)CD中带有w的抗原或抗体CDw12,CDw198,CDw199,CDw293尚需继续进行全面鉴定。
(3)有些CD抗原又可进一步划分为不同的成员,一般用小写英文字母表示,但情况有所不同:①如CD1可分为CD1a,CD1b和CD1c,这三种不同分子是分别由三个不同的高度同源的基因所编码;②CD45至少可分为CD45RA,CD45RB,CD45RC和CD45RO,它们是同一基因的不同异型/同种型(isoform);③CD2和CD2R是识别同一个分子上不同的表位;④CD49已进一步划分为CD49a,D49b,CD49c,CD49d,CD49e和CD49f,它们的基因定位于不同的染色体上,但具有较高的同源性。
(4)CD的组划分的特异性是相对的,实际上,许多CD抗原的组织细胞分布较为广泛。
此外,有的CD 抗原可由不同的分类角度而归入不同的组,如IL-2受体a链CD25是活化T细胞的标记,也属于细胞因子受体;再如某些属于T细胞、B细胞、髓细胞或NK细胞组的CD抗原实际上也是粘附分子,等等。
