改良SMILE方案治疗ENKTL-NT的临床观察

【摘 要】目的 探讨改良SIMLE方案治疗结外NK/T细胞淋巴瘤鼻型的近期疗效、不良反应和可行性.方法 2008年11月～2010年1月,2例复治、3例初治的Ⅲ、Ⅳ期ENKTL-NT患者,接受改良SMILE方案治疗.结果 5例患者中,CR 1例,PR 2例,近期总有效率60%.主要不良反应为骨髓抑制、肝功能受损.结论 改良SMILE方案对于复发难治性及晚期ENKTL-NT患者有一定疗效,但剂量、时间及用法值得深入探讨.

结外 NK/T细胞淋巴瘤鼻型(Extranodal NK/T-cell lymphoma,nasal type,ENKTL-NT)是非霍奇金淋巴瘤(non Hodgkin lymphoma,NHL)中的少见类型,发病率占 NHL的 2%～10%。Ⅰ、Ⅱ期 ENKTL-NT放疗近期疗效尚可,但远期复发率较高[1];Ⅲ、Ⅳ期多采取化疗为主、放疗为辅的综合治疗模式,但极易化疗耐药导致治疗失败。目前尚无公认的标准化疗方案。文献报道含有左旋门冬酰胺酶

(L-asparaginase,L-ASP)的SMILE方案是种有效方案[2]。2008年 11月 ～2010年1月我科尝试应用改良 SMILE方案治疗Ⅲ、Ⅳ期 ENKTL-NT患者 5例,就其近期疗效及不良反应,并结合相关文献进行分析,为其治疗提供一些参考依据。
