3、局部并发症 ❖ 急性液体积聚 急性胰腺假囊肿 ❖ 胰腺坏死 指胰腺实质弥漫或局灶坏死， 并有胰周脂肪坏死。根据感染与否又分为 无菌性和感染性胰腺坏死，CT是目前诊断 的最佳方法。 ❖ 胰腺脓肿 常在发病后4周或4周以后，由 胰腺组织坏死液化继发感染形成。

病理 Pathology
水肿、出血和坏死是急性胰腺炎的基 本病理改变 1、水肿性胰腺炎 呈局限性或弥漫性水肿、 腺体增大、被膜紧张。 2、出血和坏死性胰腺炎 腺体大片出血、 坏死，呈深红色，严重时整个胰体变黑， 大小网膜有皂化斑，腹内有血性腹水。

治疗原则：轻型急性胰腺炎一般采
用非手术治疗；重症胰腺炎则在非手 术治疗的同时，严密观察病情，对合 并坏死感染者，则需手术治疗。

临床表现 是最早出现的症状
▣上腹疼痛和饱胀不适： 早期由于胰管堵塞，管内压力增高；
晚期癌肿侵及腹腔神经丛，疼痛剧烈
▣黄疸：
是最主要的症状和体征
癌肿侵及或压迫胆总管，进行性加
重，伴皮肤瘙痒，尿色深，大便呈陶土
样，部分病人胆囊肿大

▣消化道症状：食欲不振、腹胀、消化不 良、恶心呕吐、腹泻

治疗
▅囊壁尚未成熟，<6周非手术治疗， B超、CT密切观察囊肿的变化，必要时 （迅速增大、明显疼痛、合并感染或出血）手术治
疗。 ▅囊壁已成熟，>6cm应手术治疗：
•内引流术：时间大于六周，囊肿-空肠 或囊肿-胃吻合术
•外引流术：适用于明显感染，囊壁时间 短，壁薄不能行内引流者
