Dialogue background: Johnson is a forty year old day two post-op patient who had an evacuation of a subdural haematoma and open reduction internal fixation of her fractured right humerus. Her injuries were sustained after being assaulted. She has a history of a right partial mastectomy and is a Type 2 diabetic/Niddm. At present she is a patient on a neurosurigical ward, called Five West, and has been looked after by RN John for about half an hour.

Dialogue1—Perfoming general obserations

Nurse:How are you feeling , Theresa?

Patient:I just can’t get over what has happened to me.

N:Well, that’s totally natural considering what you have been through…At handover the morning nurse said you might be interested in talking about what’s happened to you with one of our hospital counsellors?

P:Yes…I…uh…I’d like to do that.

N:Ok then, I’ll arrange for the social worker to come and visit you. Now if it’s ok with you, I need to do all your observations and your blood sugar?

P:Sure.

N:I’ll just put this thermo meter into your ear, it might be slightly uncomfortable but it will only take about one or two seconds(N uses a

tympanic thermometer) … Thirty seven point five, your temperature is just slightly high. Can I have your wrist please, so that I can take your p ulse? … Thanks. Your hear t rhythm is a little fast and irregular. Do you have a history of atrial fibrillation or any other kind of heart rhythm problems?

P:No, not that I know of.

N:You don’t need to worry about it, the rate is 110 which is not overly fast, but I’ll soon let the doctor know and see if he wants me to do an electrocardiograph on you.

P:Alright.

N:Before I take your blood pressure, could you please tell me if you have previously had any surgery done on either your chest or arms?

P:Yes…Oh!I forgot to tell the doctor, but I’ve had a partial mastectomy on my right breast.

N:Don’t worry it’s normal to have a bit of memory loss after a head injury …W ell, seeing that your right arm is fract ured I’ll need to check your BP on t he left side in any case. Please extend your arm out … When I inflate the blood pressure cuff it’ll be a little tight … It’s 145/85 which is just slightly high. Do you have any history of high blood pressure at all?

P:No, none at all.

N:Do you feel any paint at present?

P:Both my head and arm are just a little sore , but the painkillers are working well.

N:Ok, well that’s good to hear Theresa … Now I’m going to check your oxygen level, please put one of your fingers into this probe( N counts patient’s respiratory rate whilst waiting for the oxygen saturation reading) …Your oxygen level is 86%, that’s a little bit lower than normal, do you feel short of breath at all?

P:Yes John, just a little bit and I seem to be developing a bit of a cough. More and more yellow phlegm is coming up, see……(P shows the nurse a few used tissues that are in her hand).

N:Yes, quite a bit there. I’ll call the doctor to come and listen to your chest and in the meantime, I’ll turn up your oxygen a little. Oh, and here is a paper bag you can use to dispose of all your tissues (N sticks a paper bag on the patien t’s bedside trolley).

P:Thanks.

N:My pleasure.

Dialogue 2—Performing neurovascular, blood sugare and neurological observations

P:John, I’m a little worried that my vital signs aren’t normal, am I going to be ok ?

N:I can really understand that you feel anxious about things, but most of

your observations are only slightly out of the normal range. For someone who has experienced what you have these last few days, you are recovering quite well.

P:Good, that’s a relief to hear.

N:By the way, if you are brewing any kind of a chest infection it is even more important to do those half hourly deep breathing and coughing exercises that the physio taught you earlier on. You also need to drink more water and try and relax as much as possible. The sooner we get you sitting up in the chair, the better for your lungs too …There’s certainly a lot you can do to control your own recovery and ensure that it’s s mooth as possible.

P:Ok, I’ll make sure I follow your advice and I have already been using that breathing exercise thing with the plastic balls in it (incentive spirometry device) quite often.

N:Very good. Now I need to do your circulation observations. I’ll just take a quick look at your fingers ( N starts to perform neurovascular observations.) … your circulation looks fine. Can you move your fingers around please? …Good. Can you feel me touching them?

P:Yes.

N:Do you feel any tingling or numbness at all?

P:No, I don’t.

N:I’ll just do your blood sugar again…Could I please have a finger from

your left hand? …You’ll just feel a small prick on the finger …Ok your blood sugar is seven, that’s fine.

P:Good.

N:Ok, lastly I’ll just do your neurological observations. I’m sure you are sick of answering these questions, but I’ll need to ask them again, what’s your full name?

P:Theresa Elizabeth Johnson.

N:And can you tell me what day of the week it is, what date, month and year?

P:Today is Friday, right?

N:Actually it is Monday, but don’t worry, it is not unusual when someone has come into hospital, especially after a head injury, to get the days mixed up.

P:Ok, but I’m sure it’s March, somewhere around the middle of the month, and it is 2007.

N:Yes spot on, indeed it is March the sixteenth 2007. Can you now please squeeze my hands as hard as you can with both of your hands? (N gestures forward with two fingers on each hand stretched out) …That’s good…Now stretch both arms out straight in front of you, level with your shoulders, please hold them up for fifteen seconds and close your eyes…Good…And just push your feet against my hands…Ok, nice and strong…Now please bend your knees slightly.

P:Like this?

N:Yeah, exactly like that. Now I will push down and I want you to resist me as hard as you can. Ok, on the count of there. One, two, there…Great, your limbs are all very strong.

P:At last, some good news!

N:Yes! …Finally, I just want to shine t his little torch into your eyes, it is quite bright …Your pupils are equal in size and reacting normally to the light. Overall, you are doing very well but we must working together to get your chest right.

P:Right, can I sit in the chair as soon as possible?

Dialogue 3—Reporting observations to a medical officer

N:Hello, Five West, John speaking.

DR:Hi, John, it’s Tony, you paged me、

N:Yes. Hi Tony. I just wanted to update you about a couple of issues relating to Theresa Johnson in bed five.

DR:Sure, go ahead.

N:I’ve just done all her obs and while her GCS is fine at fifteen & her right arm is neurovascularly intact, she was desaturating to eighty six percent on two litres of oxygen via nasal prongs and tachypnoeic at a rate of twenty eight. She only has a low grade fever of thirty seven five, but is complaining of an increasingly productive cough.

DR:Ok, have you turned up the oxygen?

N:Yes, she is now saturating at ninety percent on four litres of oxygen via nasal prongs. In addition to this her heart rate is one hundred and ten and is quite irregular. She denies any history of arrhythmias and in her baseline ECG she was in sinus rhythm at a rate of seventy. She is mildly hypertensive at one hundred and forty five on eighty five, but says her pain is fairly well controlled.

DR:Well, her chest x-ray was clear on admission to ED(Emergency Department) but it sounds like she may be brewing a chest infection. I’ll be up there to review her chest and those other issues in about five minutes. Could you please get a sputum specimen off her, and do a 12 lead ECG. Can you also call for a mobile x-ray; I should be there to give them the request form before they arrive.

N:No problem, how about I put a Hudson mask on her too?

DR:Yes, good, please titrate the oxygen flow to maintain her O2sats above 95%.

N:Will do, see you soon then.

DR:Ok, thanks.

Dialogue background: Anne is a fifty year old woman who has been admitted to the respiratory ward, Three South, for an infective

exacerbation of her chronic obstructive pulmonary diseas (COPD). She has been a heavy smoker of both cigarettes and marijuana for approximately thirty years and has not been able to cut down at all. Her medical history also includes multiple sclerosis, mild chronic renal failure, deep vein thrombosis, depression and being on long term prednisone for her COPD. She openly identifies as being gay and is often visited by her partner, Sharon. At present she becomes shrt of breath at the slightest level of exertion and is very weak, listless and depressed; she is dependent on low flow oxygen and intermittent BIPAP. RN Julie admitted Anne eyesterday and has been allocated to care for her again today.

Dialogue 4—Assisting a patient with eating and drinking

N:Anne, here is your dinner.

P:I don’t feel like eating.

N:Do you feel sick or uncomfortable in anyway?

P:No, I don’ t feel sick, but I hate the food and just feel so short of breath and weak. I just don’t have any appetite at all.

N:I can really understand that you don’t feel too hungry with all your discomfort at present, but your body needs the nutrition so that you can regain your strength and energy. Perhaps you could just try to eat a small amount? …Please let us know if there is something you particularly feel like eating.

P:No, there really isn’t anything at all that I feel like eating…ugh…Ok, I’ll try and eat a little bit.

R:I’ll cut the meat into smaller pieces and open things up for you.

P:Thanks.

N:My pleasure…Will you be ok feeding yourself?

P:Yes, I think so…(P eats some of her meal)

N:Are you finished your meal or would you like some more time?

P:Finished! No more please!

N:I’ll take your tray away then, ok?(P nod s) …I could see that was hard work for you, but you did really well, you ate half of the meal! …What time will Sharon come in today?

P:It should be pretty soon; as soon as she has taken the dogs for a walk. N:What kind of dogs are they?(N makes general conversation with the patient in order to put her at ease)

P:Labradors.

N:It’s hard to find a more loyal dog than a Labrador.

P:Yes, we love them…spoil them like they were children…they are quite naughty at times though.

N:Well I’m sure they are still a lot more obedient than my children are! I better not make you talk too much or you will get more short of breath…You look a little dehydrated, do you think you could try to drink more water too?

P:Could I have a tea or coffee instead?

N:Sure, one isn’t a problem. Overall though, you really need to drink more water, or at least fruit juice or cordial to replenish your fluids. Did you know that coffee and tea are mild diuretics that make you urinate and lose more fluid?

P:No I didn’t. Ok, you can give me some orange cordial then.

N:No problem, I’ll fill your jug of water up and go and get you some cordial.

P:Thanks.

N:Because you are so short of breath I think we better give you a bed sponge instead of a shower today, what do you think?

P:Definitely! But, I need to open my bowels now and I can’t do that in bed on a pan, it’s just too uncomfortable.

N:If you use the commode by the be dside, I’m afraid that transferring across to it will make you too short of breath.

P:I realize that, but no matter what, I’m going to use the commode! I’m not toileting in bed!(P tone of voice is slightly angry)

N:I can hear that it’s really important for you to toilet out of bed…Ok, how about I get the hoist and we can use that to transfer you across to the commode?

P:Yes, that would be good.

N:Instead of sponging you in bed we may as well give you a quick wash while you are sitting on the commode. The extra time sitting up out of bed will be good for your lungs. Is that ok with you?

P:Yes alright, but I want to go straight back to bed after the wash.

N:No problem, of course…I’ll just go and get the commode chair and the hoist and try to find our student nurse, Alan, to give us a hand with the transfer. I’ll be back in just a tick.

Student Nurse:Hello, Ms Kosta skidis, I’m Alan. How are you?

P:I’ve been better days Alan!

SN:Yes you certainly look short of breath…I’ll draw the curtains.

N:First I’ll attach so me extension tubing to your nasal prongs(N uses longer O2tubing so the nasal prongs can reach the oxygen port) …Now we just need the pop this harness underneath you. Can you please roll over to your right…now your left please…I’ll just connect the hoist u p the harness…Are you ready?

P:Yeah.

N:Ok here we go (N uses the hoist to transfer P from bed to commode chair) …Are you ok, do you feel steady on the commode ( P nods ) …Here is the buzzer; you can buzz us as soon as you are finished…(5 minutes later P presses her buzzer)…I’ll just get your toilet paper and help you wipe your bottom, ok?

P:How embarrassing!

N:I can understand you feel that way, but it’s important you conserve your energy at present, and it’s not just you that needs help with this; a lot of patients we look after need help with toileting… Now I’ll just wash my hands and make your bed and Alan will give you a sponge.

SN:Ok Anne, just quickly dip your hand in this bowl of water. Is the temperature right or is it too hot or cold?

P:No, that’s j ust right.

SN:I’ll just get your face washer and soap from your drawer…Ok, lets start with your face and neck…And I’ll wipe you dry…So have you lived in Sydney all your life? (SN proceeds to make conversation with P, but

is mindful of not making her too s hort of breath ) …Ok I’ll just take your Ted (TED) stockings off and wash your feet and then we are done.

N:Do you feel clean enough Anne? ( P nods ) …Here’s a fresh gown…I’ll transfer you back into bed now… ( Nurses take harness out ) …Alan do you mind helping Anne with her teeth and hair? …Anne I’m just going to clean your catheter( P has a Supra Pubic Catheter) insertion site with some saline, ok?

P:Ta.

SN:I’ll put your Ted (TED) stockings back on…Would you like to brush your teeth? ( P nods) …Here is your toothbrush with some toothpaste on it…Here is a bowl and a cup of water…( P spits toothpaste into bowl) …Here’s your comb…Anne would you like some deodorant on? …Would you like some of your make up?

P:Yes, please give me my lippy ( colloq. for lipstick).

SN:Here you go…Julie, do you want me to give Anne’s ventolin (salbutamol) and atrovent (ipatromium bromide) nebuliser that is due for nine o’clock?

N:Yes, that would be great thanks Alan. I’ll wait here, so that I can check it for you and I can start setting up her BiPAP for afterwards too. (SN goes to the treatment room to get the medications)

Dialogue 6—Dicussing patient’s dietary and other issues with a doctor

DR:I just got Anne Kostakidis’s EUCs/UEC’s back and her potassium, urea and creatinine are going up. So, we need to put her on a renal diet. Also, when I examined her this morning I found that her peripheral oedema and JVP are also getting worse; she needs to go on a one litre fluid restriction too please.

N:Robert, she really is struggling to eat as it is. I really don’t think she will comply with a diet that is even more bland than what she is getting now. She ate half of her breakfast, but her intake is really erratic and she will often entirely skip meals. I’ve only just weighed her and a ccording to her partner, Sharon, she has lost five kilo’s in the last two weeks. They forgot to mention this to the ED (emergency department) doctor because Anne was so dyspnoeic when she first came in.

DR:Right. So, why isn’t she eating?

N:Well Sharon said that of late Anne has just been so short of breath and has become more and more lethargic and anergic. She also suspects that Anne’s depression is an issue too, and I’m guessing that this is currently being further exacerbated by her withdrawal from cigarettes and THC. Not to mention the fact that her chewing ability is slightly compromised due to her MS.

DR:Yes, she certainly smokes enough of both cigarettes and marijuana to

be experiencing some withdrawal…With her anorexia and weight loss though, while it’s an unlikely diagnosis, we shouldn’t totally rule out the possibility that they are related to a malignancy. In the meantime could you please get the dietitian to come and review her? Perhaps she can advise us on some kind of renal diet that Anne will find more agreeable. Otherwise she is looking towards needing either NG feeding or TPN, but of course neither of those options would be considered particularly ideal solutions in her situation.

N:Sure, I’ll call her in a minute. Robert, she already looks q uite dehydrated and her urine output is minimal and quite concentrated. How is she going to handle the fluid restriction?

DR:Yes, it’s a catch 22 situation, but if she keeps retaining fluid she’ll end up needing dialysis or in pulmonary oedema, so for the time being we don’t have a choice.

N:Ok. I was also going to call the drug and alcohol CNC to have him review her addiction and withdrawal issues, and the psych liaison CNC to come and assess her mood. I’ve asked Anne and she is quite happy to talk to them and wants Sharon to be present too.

DR:Good idea. We can offer her NRT if she wants it too ( N nods ) …One more thing Julie, we need to increase her prednisone again to seventy five milligrams, so could you please increase her BGLs from daily to QID?

N:S ure, I’d better go and make all those calls now! See you later. DR:Ok, see you Julie.
