员工疲劳综合征(Staff burnout)是当今医疗服务队伍中的一个重要问题[1]。疲劳综合征是一个发展的过程,它与长期过重的工作压力有关,这种压力导致员工心理紧张,产生躯体、情绪和心理的症状及体验,最终出现逃避工作的行为。据报道,护士易患疲劳综合征,但目前病因尚未明确,争论也较多,可能与护士所承受的巨大专业压力及心理健康状况有关[1,2]。本文对肿瘤医院护士疲劳综合征状况及相关因素进行调查分析,以探讨它们之间的相关关系,为保证护士身心健康提供依据。

1 调查对象与方法

1.1 调查对象采用整群抽样调查的方法,对广东省中山大学肿瘤防治中心和汕头大学医学院肿瘤医院的257名护士进行调查分析。

1.2 调查工具

1.2.1 抑郁自评量表(SDS)和焦虑自评量表(SAS)[3]:量表各有20个条目,应用1—4级评分法。

1.2.2 护士工作压力源量表[4]:量表稍作改动,由5个方面36项子条目组成,采用1—4级评分法,统计均值和单因素百分率。分数越难,则压力越大。1.2.3 员工疲劳综合征量表:根据WHO编撰的“员工疲劳综合征”主要躯体、情绪和心理的症状及体验,在心理学教授的指导下自行设计,具有较高的信度和效度。

1.3 方法应用自评问卷,规定统一指导语,由护理部发放调查。本研究共发放问卷260份,回收有效卷257份,占98.8%。全部测评资料量化后建立数据库,用SPSS11.0统计分析软件包进行u检验和多元逐步回归分析。

2 结果

2.1 焦虑和抑郁状况调查结果(见表1)

测评结果显示,压力源5个方面依次为:①护理专业及工作方面的问题,如认为晋升和深造的机会太少(82.9%),高风险职业压力(78.6%),社会地位太低(71.6%),职位竞争压力(66.9%),经常倒班(60.6%);②工作量及时间分配问题,如超负荷工作(78.6%),非护理性的工作太多(61.1%);③病人护理方面的问题,如担心医疗护理纠纷(80.5%),担心差错事故(79.8%),所学知识不能满足病人需要(78.6%),病人及家属不合作不礼貌(76.7%)和要求太高(75.1%),护士工作未被认可(74.7%),以及护理晚期病人的压抑感(65.0%)和对生离死别、悲伤的情感反应(65.8%)等造成护士较高的工作压力;④管理及人际关系方面;⑤工作环境及资源方面的问题,对护士影响较小。

以护士疲劳综合征总分均值为因变量Y,职业压力源5个方面及总分均值为自变量X1-X6,SDS和SAS评分为X7和X8进行多元逐步回归分析。得回归方程(略)

3 讨论

3.1 肿瘤医院护士焦虑和抑郁状况分析

护理工作的性质及特点决定了护士的职业压力[4]。研究表明,多数人在面对职业压力时会出现身心的紧张性反应。适当的紧张状态有利于动员机体的潜在能力去适应,如果这种反应过分强烈或者持久,超过了机体的调节能力,则可产生焦虑、抑郁等紧张情绪或出现其他病理征象,最终将导致影响护士的身心健康及他们所提供的护理质量。

调查结果显示,肿瘤医院护士焦虑和抑郁评分均值明显高于国内常模,抑郁反应发生率为30.4%,明显高于我国正常人群15.1%-22.5%的报道。分析认为,护士每天围绕着服务对象的精神、心理、生理及社会问题进行工作,而解决问题的方法又常常是不容易获得的,因而工作场景难免令人沮丧。在肿瘤科还需常常面对难治性或预后不良的癌症病人、临终病人及其家属,这些工作本身就十分容易使护士感到疲惫。他们可能“因心理能量在长期奉献的过程中被逐渐耗竭,而产生身心极度疲惫和感情的枯竭”[9]。研究提示,肿瘤医院护士的心理健康不容忽视。

3.2 肿瘤医院护士职业压力源分析

本组资料表明,肿瘤医院护士存在较高的职业压力,护理专业及工作方面的问题、工作量及时间分配问题及病人护理方面的问题是主要的职业压力源,而管理及人际关系方面和工作环境及资源方面的问题对护士的影响较小。护士的社会地位仍较低,超负荷的工作使护士身心疲惫,但又常常未被尊重和理解。临床工作中,护士与病人接触最多,护患之间由于涉及双方的权益,健康、经济、人格及法律问题,稍处理不当,常可引发冲突,给护士带来一定的心理压力。医疗卫生行业本身就是一个高风险职业,日益突出的职业暴露、职业防护问题,医护、床护比例失调等等问题,无疑增加了护士的职业源性压力。调查资料还显示,长

期护理难治性肿瘤病人的压抑感和救治垂危病人失败的内疚,交织着悲伤、生离死别的抑郁情感体验等,也给肿瘤科护士造成较大的负性精神刺激,导致紧张性反应的不断累加,直接影响身心健康。

3.3 肿瘤医院护士疲劳综合征状况分析(略)

综上所述,护士由于职业特性易患疲劳综合征,应引起有关管理者重视。医院领导层在强调提高临床护理质量的同时,应注重临床护士的行为心理管理,协助护士正确认识和舒缓压力,促进身心健康,保证护理质量。本研究资料来源于问卷调查表,可能受个体因素、人文社会环境的影响,特别是缺乏对研究对象的追踪和动态监测,在一定程度上可能影响研究结果。能否在纵向研究的基础上,应用某些生物学指标作为对调查表的一种客观补充方法,以使研究项目更趋完善,值得进一步探讨。
