Dear Sir or Madam,
I am Li Hua. I am sorry to trouble you but I am afraid that I have to make a complaint. The reason why I write to you is that the poor service of a waiter called Li Ming made me dissatisfied.
I had already booked/reserved a seat in advance a few days ago, but it was he who forgot to keep the seat for me so I had to wait outside for half an hour. In addition, I was not told that the prices of some of the dishes had increased until I finished my meal. Worse still, when I was ordering my dishes, he couldn’t answer my questions patiently. The most astonishing thing is that he was so careless that he made a mistake on my bill.
Not only did he make many mistakes but he did not serve us customers well. Therefore, I want you to teach the waiter and give him proper punishment.
