Case in point: researchers from the University of Michigan reported in the New England Journal of Medicine yesterday that 42 percent of Americans over the age of 60 end up having to make some kind of decision about their medical care and, of those, 70 percent are incapable of doing so. These are impressive stats and make advance directives – which outline a person's wishes for end-of-life care –potentially very useful. Dr. Kenneth Langa, the paper's senior author, says he and his colleagues started their research several years ago, but got even more motivated as the Death Panel debacle ensued. "Last summer's debate was incredibly saddening and really aggravating for people who work with patients at the end of life and see the kinds of incredible emotional struggles that these kinds of decisions place on patients and their families," says Langa. "It made us redouble our efforts to get this research done."The reality is that patients want to have a say in what happens to them when they're sick and, more often than not, they don't want heroic and often hugely costly measures to save them. In the new study, researchers found that more than 90 percent of the adults who had living wills requested either limited care or "comfort care" at the end of life. Only 1.9 percent (a total of ten patients out of 3,746) asked for "all care possible". Aggressive medicine does not equal happy patients. Doctors must acknowledge this and have honest and informative conversations with their patients. In another study, published in 2008, researchers found that end-of-life discussions resulted in less aggressive care – including ventilation and resuscitation – and earlier hospice enrollment, which equaled better quality of life for patients.Healthcare organizations need to support hospice and palliative care. And doctors, nurses and other healthcare professionals need to be trained in advance care planning.
