软组织肿瘤最新分类 WHO

《软组织肿瘤病理诊断免疫组化指标选择专家共识》要点软组织肿瘤是外科病理学中一类病种最多，也最为复杂的肿瘤。

WHO(2013）软组织肿瘤病理新分类包括12大类。

各类肿瘤又包含很多种疾病类型，并根据生物学行为的不同，分为良性、中间性和恶性。

与其他类型的肿瘤相似，软组织肿瘤的病理诊断也需与临床、病理相结合。

除部分肿瘤可根据临床特点和镜下形态直接做出诊断外，大多数软组织肿瘤需加做免疫组化标记。

1 免疫组化在软组织肿瘤病理诊断中的应用及注意事项免疫组化不仅在软组织肿瘤的诊断和鉴别诊断中起非常重要的作用，而且在指导靶向治疗或预测肿瘤的生物学行为等方面也有广阔的应用前景。

但要强调的是，免疫组化只是一种辅助性手段，有其自身的局限性，并不能代替传统的组织学检查，后者才是病理学诊断的基础。

免疫组化检测必须以病理组织学形态为基础，选择免疫组化检测指标时应注意以下几个问题：⑴熟悉常用抗体的反应谱和适用条件，首选敏感和特异性较高的抗体类型，特别是公认的抗体型号，并合理配伍，力争采用尽可能少的抗体取得最好的检测结果。

⑵免疫组化检测过程需注重质量控制，尽可能做到标准化，使染色切片背景清晰，标记定位准确。

⑶推荐设置阳性对照，尤其是一些与靶向治疗密切相关的标志物，如CD117等，以确保免疫组化标记结果的可信性。

⑷对标记结果要注意辩证分析，因有相当一部分抗体在一些不同类型的肿瘤之间存在交叉反应或有异常表达，如S-100蛋白在滑膜肉瘤中的阳性率也可高达38％，因此不能仅根据S-100蛋白标记阳性简单地将梭形细胞肉瘤诊断为恶性周围神经鞘膜瘤。

CD31不仅表达于血管肉瘤，也可表达于组织细胞肿瘤。

另外，HMB45是恶性黑色素瘤、软组织透明细胞肉瘤和血管周上皮样细胞分化的肿瘤（PE-Coma）的标志物，但最近有文献报道部分子宫平滑肌肉瘤和子宫内膜间质肉瘤也可表达HMB-45。

⑸软组织肿瘤中存在一些异常表达的情况，如标记上皮细胞的角蛋白，也可在假肉瘤样肌纤维母细胞性增生、胚胎性或腺泡状横纹肌肉瘤和骨外尤因肉瘤等一些不具有上皮样分化的软组织肿瘤中表达。

新版(2020)WHO骨肿瘤分类解读刘斯润*，蔡香然，邱麟WHO 第5版骨肿瘤分类已于2020年出版，对照2013年的第4版WHO 骨肿瘤分类(表1)，本文根据新版疾病分类的变动和知识的更新进行简单分析归纳，对新增疾病的定义、流行病学、病理学特点、临床症状及影像学特征进行简单剖析，以期加深对新版骨肿瘤分类的理解和认识。

1 整体比较2020新版WHO 骨与软组织肿瘤分类有4部分内容：①软组织肿瘤；②骨与软组织未分化小圆细胞肿瘤；③骨肿瘤；④遗传性骨与软组织肿瘤综合征。

其中2、3、4部分与骨肿瘤相关。

其中的骨肿瘤分类，对比第4版有更简洁、科学和方便应用的特点。

总体上大的分类由原来12类减少为8类，共包含了68种疾病。

删除了纤维组织细胞性肿瘤、肌源性肿瘤、脂肪源性肿瘤、未明确肿瘤性质的肿瘤、杂类肿瘤5大类的名称，并将其中一部分疾病重新归类，大部分归入到其他间叶性肿瘤分类中。

此外，2020新版还新增、删除或重新划分了部分疾病；如尤文肉瘤被划分到骨和软组织未分化的小圆细胞肉瘤中；丰富了一系列骨的造血系统肿瘤名称；新增了遗传性骨与软组织肿瘤综合征；2020新版继续延用上一版良性肿瘤、中间型(局部侵袭/偶见转移)和恶性肿瘤的划分方法，但对部分肿瘤的生物学行为重新进行了划分。

2 分类对比介绍2.1 软骨源性肿瘤2020新版软骨源性良性肿瘤中删除了软骨瘤，滑膜软骨瘤病简化更名为软骨瘤病并升级为中间型，保留了极为罕见的骨软骨黏液瘤。

把上一版软骨黏液样纤维瘤和软骨母细胞瘤从中间型降级为良性，把同处于上一版中间型的非典型软骨肿瘤/软骨肉瘤Ⅰ级分开，非典型软骨肿瘤仍处于中间型，而软骨肉瘤Ⅰ级归入恶性病变中。

软骨母细胞瘤在2013版中为中间型(偶见转移型)，而在新版中归入良性中，由于80%以上的该肿瘤可通过手术刮除而成功治愈。

有10%～18%的病例可复发，发生于长管状骨者复发率低，发生于扁骨或颅面骨者复发率高，并无特异的组织学征象能预测局部复发。

第五章软组织肿瘤的病理鉴别诊断第一节概述一.软组织肿瘤的定义、命名和分类软组织是人体分布最广泛的组织，可定义为除上皮，实质器官及其结缔组织支架，淋巴造血组织，脑膜及神经胶质之外的所有骨外组织。

包括纤维组织，脂肪组织，肌组织，脉管，滑膜间皮等间胚叶组织。

凡起源于上述组织的肿瘤统称为软组织肿瘤。

软组织肿瘤的命名基本上可根据其生物学行为分为良性的“瘤”和恶性的“肉瘤”。

命令时再冠以起源组织的名称，如脂肪瘤和脂肪肉瘤；平滑肌瘤和平滑肌肉瘤。

软组织肿瘤的分类，主要依据肿瘤组织及其细胞形态，并辅以特殊染色、免疫组化，分子生物学，电子显微镜等其它辅助诊断枝术。

1.共有近50位专家，数百十位有关贡献者及参考2000余篇相关参考文献编写而成，共309页之多，大多是病理的描述。

2.对女性生殖器官组织学分类取得基本一致的共识，对世界各国病理组织取得统一标准，利于正确诊断和有可对比性，对临床医生正确认识良性、交界性、原位癌和癌症及其后续处理，防止过度或治疗不足或人性化的保护妇女生殖健康等均有积极作用。

3.也与国际抗癌协会肿瘤分期（TMN ）、国际妇产科联盟（FIGO ）分期等相对应可供参考。

4.第4版分类中包括卵巢、腹膜、输卵管、子宫韧带、子宫体、妊娠滋养细胞疾病、子宫颈、阴道和外阴九大部分分述，并非按女性生殖道部位顺次排列。

5.新版卵巢浆液性癌诊断釆用低级别和高级别二级分类法，取消以前高，中，低分化三级分类法；卵巢交界性肿瘤命名尚存争议，增加交界性肿瘤和非典型增生性肿瘤命名；浆液性交界性肿瘤有微乳头亚型，其肿瘤直径＞5mm 融合区的微乳头结构，且细胞的非典型变化明显，易出现腹膜种植；交界性肿瘤成分应超过肿瘤的＞10%，不足者仍列入良性中；交界性肿瘤的微小浸润灶最大直径＜5mm ；卵巢生发上皮包涵囊肿大小为＜1cm ，否则为浆液性囊腺瘤；子宫内膜样交界性肿瘤腺体融合生长，膨胀浸润＞5mm 或明显浸润应诊断为癌等，卵巢肿瘤中还有一些其他改变。

6.子宫体肿瘤中对子宫内膜增生分类、子宫内膜上皮内癌、子宫间叶肿瘤涉及子宫肌瘤、肉瘤等命名和归类有改变。
