功能状况。
本文发现高血压患者左右心室舒张功能指标E /e ’均升高外,余舒张指标E /A 、1/3FF 、e ’/a ’均下降。
高血压患者体循环系统血压升高不仅损害左室舒张功能同时导致右心室纵向的舒张功能受损,这就是心室间的相互作用造成的。
上述因素等不仅作用左室同时也作用于右室,所以对于轻、中度高血压患者常规检测左室舒张功能指标即可反映右室舒张功能的状况,这为简便操作评价高血压患者舒张功能提供了依据。
3.2心脏舒张功能是指心肌扩张和容纳血液的能力早期舒张功能异常的辨别对高血压的治疗及预后有重要意义。
二尖瓣、肺静脉血流频谱的变化和左房做功的增加、左房构形的改变、Ea /Aa (二尖瓣环舒张早期运动速度e 与舒张晚期运动速度a 比值)的降低,都反映了左室舒张功能的异常。
在高血压早期,心脏舒张功能受损尚处于代偿期,表现为左室主动扩张功能障碍为主,心肌僵硬度增加,快速舒张功能下降,血流流入心室受阻,左室舒张早期充盈量减小,充盈时间延长。
多表现为二尖瓣EF 斜率减慢[5]。
本研究发现高血压病组左室舒张功能多项指标与正常组相比,有显著性差异。
这些变化多因临床症状不典型易被忽视,所以应用超声技术检测,可早期发现舒张功能受损,从而指导临床。
3.3心室舒张功能指标优劣比较及应用价值本文探讨中运用二尖瓣血流频谱和组织多普勒技术相结合。
心室舒张功能的研究日益受到重视,左心室舒张功能异常主要导致心室充盈受损或左心室舒张末压(LVEDP )增高,LVEDP 公认为是评价左心室舒张功能的金标准[6]。
用二尖瓣血流频谱指标E /A 评价心室舒张功能较简捷,但易受心脏负荷、心率、心律等影响,并且不能区分正常和假性正常化(左心室充盈压升高)[7];而组织多普勒技术测定舒张功能指标较少受心脏负荷影响[8],其中e 波降低是舒张功能不全的最早期指标之一,且e 波降低存在于舒张性功能不全的所有阶段。
本研究二尖瓣前向血流频谱E 峰与组织多普勒二尖瓣环测e 峰比值E /e ,在高血压病组与正常组差异具有统计学意义,可能与高血压导致左心室肥厚及左心室心肌纤维化有关。
