第二节检定证书、校准证书和检测报告一、证书、报告的分类各类检定、校准、检测完成后，应根据规定的要求，以及实际检定、校准或检测的结果，出具检定证书、检定结果通知书、校准证书（校准报告）、检测报告。

证书、报告应有规定的格式，使用A4纸，用计算机打印。

要求术语规范、用字正确、无遗漏、无涂改、数据准确、清晰、客观,信息完整全面、结论明确。

证书、报告经检定、校准、检测人员、核验人员、签发人员签字,加盖公章后发出。

对各类不同的证书、报告有如下特殊要求：（一)检定证书和检定结果通知书凡是依据计量检定规程实施检定的，检定结论为“合格”的出具检定证书.每一种计量器具的检定证书应符合其计量检定规程的要求。

证书名称为“检定证书”。

其封面内容包括:证书编号、页号和总页数;发出证书的单位名称；委托方或申请方单位名称；被检定计量器具名称、型号规格、制造厂、出厂编号;检定结论（应填写“合格”或在“合格”前冠以准确度等级）；检定、核验、主管人员用墨水笔签名；检定日期：X年X月X日；有效期至：X 年X月X日。

检定证书的内页中应包括如下内容：每页的页号和总页数；本次检定的原始记录号；本次检定依据的计量检定规程名称及编号；本次检定所使用的计量标准器具和配套设备的名称、型号、编号、检定或校准证书号(有效期）、技术特征(如准确度等级、量值的不确定度或最大允许误差）；检定的地点（如本实验室或委托方现场)；检定时的环境条件(如温度值、湿度值);检定规程规定的检定项目(如外观检查、各种计量特性、示值误差等)的结论和数据。

如果检定过程中对被检定对象进行了调整或修理，应注明经过调修，并尽可能给出调修前后的检定结果。

还应包括检定规程要求的其他内容。

检定证书内容表达结束，应有终结标志。

当检定结论为“不合格”时，出具证书名称为“检定结果通知书"。

其结论为“不合格”或“见检定结果”，只给出检定日期，不给有效期，在检定结果中应指出不合格项。

其他要求与“检定证书"相同.（二)校准证书凡依据国家计量校准规范，或非强制检定计量器具依据计量检定规程的相关部分，或依据其他经确认的校准方法进行的校准，出具的证书名称为“校准证书"(或“校准报告”）。

仪器设备检定校准证书确认表(范本)
XXX已获得XXX颁发的ZC3-A型混凝土回弹仪检定证书。

该证书确认了以下项目的检定结果：指针摩擦力、指针长度、弹击端部球面半径、弹击拉簧刚度、弹击拉工作长度、弹击拉伸长度以及钢砧率定值。

检定日为2019年6月13日，检
定周期为6个月。

在JJG817-2011的指导下，XXX对ZC3-A型混凝土回弹
仪进行了检定。

检定结果表明，所检定项目符合有关标准的要求。

具体检定数据如下：指针摩擦力为0.68N，指针长度为20.02mm，弹击端部球面半径为24.0mm，弹击拉簧刚度为782.3 N/m，弹击拉工作长度为65.56mm，弹击拉伸长度为
75.06mm，钢砧率定值为80.4.
根据校准要求值，XXX确认了以下项目的校准结果：指
针摩擦力、指针长度、弹击端部球面半径、弹击拉簧刚度、弹击拉工作长度、弹击拉伸长度以及钢砧率定值。

校准结果表明，所有项目均符合要求。

校准要求值为：指针摩擦力
0.65±0.15N，指针长度20.0±0.2mm，弹击端部球面半径
25±1.0mm，弹击拉簧刚度785.0±30.0N/m，弹击拉工作长度61.5±0.3mm，弹击拉伸长度75.0±0.3mm，钢砧率定值80±2.
因此，XXX的ZC3-A型混凝土回弹仪检定结果符合相关标准要求。
