关于Casereport,你知道多少真相不会做实验,流行病学也不懂,meta好像很难的样子,在魔都三甲医院快被逼疯了,好像case report是我的唯一出路了。
真的么?让我们来八一八case report那些真相。
case report字数少,工作量小,写起来简单,轻松。
因此倍受很多国内作者的青睐,但这只是一个看起来“简单”的事情,实际“不简单”。
主要原因有三:1、绝大多数杂志发表case report的标准是:该类case report没有人报道过;该case report对本专业的知识有重大贡献。
前一个标准好确定,到PUBMED上查一下就知道了。
第二个标准比较主观,不好确定,什么是“重大贡献”,不同的editors可能有不同的标准,有时候是否能够被接受完全取决于editors当时的心情;2、杂志给case report版面远少于给researcharticle的版面。
绝大多数杂志给case report的版面很有限,每期就1-3个cases的版面。
更有甚者,有些杂志根本不接受case report,有些杂志说:由于已接受的case report太多,2年内发不完,故近2年不接受casereport投稿。
至于为什么杂志case report版面少于researcharticle,是因为case report不易被引用,影响杂志社的IF 值,杂志社也是要活的嘛。
3、向杂志投稿的case report量大,因为casereport字数少,工作量小,故大多数临床作者都愿意写,愿意投,故投稿量大。
因此,发表标准有些主观,发表版面很少,但投稿量又大,造成了发表case report实际上是一件看起来“简单”但实际上“不简单”的事情。
若要投稿casereport,我们有以下建议:1、查看是否有创新性,如新发现基因位点突变,比以往报道长的20年(尤其在一些老医院多有这些资料)的随访等等,如PUBMED上查询,已经有人报道,就直接放弃。
病例报告和病例系列分析临床流行病学研究中,经常进行的病例报告和病例分析都属于描述性研究,由于缺乏严密的设计和规范的对照分析,科学性较差,论证强度低,因此,只能作为分析性研究和实验性研究的基础性研究及临床经验的总结。
但病例报告及病例分析容易进行。
临床医生面临大量临床资料和病史记录,随时都可以拿来总结分析,所需时间短,不需要很多的人力和物力就能对临床上各种问题进行研究,并可迅速提出有关线索与假设。
因此,病例报告及病例分析至今仍是临床医生应用的主要研究方法。

据Fletcher RH的统计,国外的主要医学期刊,20,~30,的原著中刊登10例以下的病例报道。
那种认为病例报告论文是价值不大的文章是毫无根据的。
尤其是对人类新发生的疾病或临床事件的首例报告,具有重要参考价值。
1985年,我国报道首例艾滋病病例;1996年,浙江医科大学附院内科报告一例鞭节舌虫病,也是世界上首例报道,是食五步蛇胆及血而感染;莱姆病等也是通过首次病例报告,才逐步认识的。
病例报告的意义是为进一步临床研究提供线索,是临床研究新思路的丰富源泉。
病例报告虽能详细描述该病例的临床和治疗经过,但由于病例是高度选择的研究对象,因而极易发生偏倚,尤其是关于治疗成功的个案报道,因为杂志编辑部一般不愿意报告治疗无效的病例。
病例报告不能估计疾病或临床事件的发生频率,也不能估计机遇的作用,如有时报告两个罕见临床事件同时存在被认为有生物学意义,而实际上是机遇所造成。
因此,对病例报告的结论要有正确的估价。
2(病例系列分析(case series analysis)病例系列分析是对发生在一个相当短的时期中,单个病例报告的集合描述与分析。
这类资料不仅对于临床信息积累、临床经验总结有帮助,而且对疾病诊断、治疗技术的研究都是很好的基础材料。
