第三节棘上及棘间韧带损伤 学习要点 1．棘上韧带和棘间韧带的损伤机制 2．棘上韧带和棘间韧带损伤的临床表现 概论 该症又称棘突骨膜炎、棘间韧带断裂或撞击性棘突滑囊炎。棘上韧带和棘间韧带位于棘突之上和棘突之间，将相邻两棘突连结在一起，因韧带位置较浅，活动时又易受到牵拉而损伤。易发生部位：腰椎4、5（44.8％）、腰5骶1(34.3％)及胸椎（7.8％）。 【损伤机制】 棘上韧带和棘间韧带均有限制脊柱过度前屈的作用。脊柱向前弯曲到一定程度，骶棘肌即完全松弛，此时完全由韧带维持脊柱的姿势，足见韧带牵张拉力之大。当弯腰劳动时，脊柱呈前屈状，棘上韧带处于外层皮下，且被拉紧，易受外力的直接损伤。如长期弯腰，使棘上韧带受到劳损，久之发生部分断裂，局部出血，肿胀而疼痛。在日常生活中，腰部的屈伸运动常使棘突分开和挤压，并相互摩擦，从而造成棘间韧带的牵拉和挤压，韧带纤维之间的经常性机械摩擦，都会引起韧带变性、断裂、出血或渗出。 1．运动员背伸动作练习过多，棘突反复撞击使韧带受伤。排球扣球、跳水折腰、体操后软翻等，极易造成此伤（图2－3－1，2）。 图2－3－1 跳水棘突损伤

阿是穴温针灸加针刺后溪穴治疗棘上、棘间韧带损伤的临床研究 发表时间：2013-06-08T09:18:10.297Z 来源：《医药前沿》2013年第12期供稿作者：何洪洲1 邹小凤2 彭俊铭1 冯淑兰3（指 [导读] 督脉经气通，血行畅，阳气旺，则肌肉得于温煦，筋骨得于濡养，“通则不痛”。 何洪洲1 邹小凤2 彭俊铭1 冯淑兰3（指导） (1广东省揭阳市揭西县人民医院广东揭西 515400) (2广东省揭阳市揭西县中医医院广东揭西 515400) (3广州中医药大学第一附属医院广东广州 510405) 【摘要】目的观察阿是穴温针灸加针刺后溪穴治疗棘上、棘间韧带损伤的临床疗效。方法采用随机对照试验的方法将120例患者分为2组各60例，治疗组采用阿是穴温针灸加针刺后溪穴法，对照组采用齐刺加推拿法，对比观察2组临床疗效。结果 2组均能治疗棘上、棘间韧带损伤，治疗组总有效率100%，治愈率90.0 %；对照组总有效率78.3%，治愈率60.0%，阿是穴温针灸加针刺后溪穴的治愈率及总有效率明显高于齐刺加推拿组。结论阿是穴温针灸加针刺后溪穴治疗棘上、棘间韧带损伤，能取得非常满意的临床疗效，且操作简单，痛苦少，值得推广应用。 【关键词】棘上棘间韧带损伤阿是穴温针灸针刺后溪穴 【中图分类号】R745 【文献标识码】A 【文章编号】2095-1752（2013）12-0120-02 棘上韧带是从枕骨隆突到第5腰椎棘突，附着在棘突的表面。棘间韧带是连接两个棘突之间的腱性组织，棘间韧带位于棘上韧带的深层。两种韧带主要是防止脊柱过度前屈，往往同时损伤。棘上、棘间韧带损伤在临床上十分常见。目前治疗棘上、棘间韧带损伤主要有局部封闭疗法和齐刺加推拿疗法。局部封闭疗法无菌技术要求较高，使用激素容易导致皮肤色素减退，肌肉萎缩，很多患者拒绝接受。齐刺加推拿疗法操作简单，安全无不良反应，治疗过程中患者无明显痛苦，临床上较为广泛应用，但其治愈率不高，故有一定的限制性。近年来，笔者采用阿是穴温针灸加针刺后溪穴治疗棘上、棘间韧带损伤取得满意疗效，现报道如下。 1 临床资料 1.1 诊断标准 参照《外科学》 [1]拟定。临床表现：多无外伤史。腰痛长期不愈，以弯腰时明显，但在过伸时因挤压病变的棘间韧带，也可以引起疼痛。部分病人痛可向骶部或臀部放射。检查时在损伤韧带处棘突或棘间有压痛，但无红肿。有时可扪及棘上韧带在棘突上滑动。棘间韧带损伤还可通过B型超声或MRI证实。 1.2 纳入标准①符合上述棘上、棘间韧带损伤诊断标准。②年龄在19～68岁之间,性别不限。③未用药物和其他方法治疗,或停止原治疗两天以上,临床表现仍符合诊断标准者。④知情同意者。 1.3 排除标准①不符合棘上、棘间韧带损伤诊断和纳入标准者。②腰椎骨折与脱位、骨质疏松、腰椎间盘突出症、腰椎滑脱、结核或占位性病变等疾病引起的腰痛者。③妊娠期妇女,并发心血管、脑血管、造血系统、肝、肾、内分泌系统等严重原发性疾病和精神病患者。 ④过敏体质及对多种药物过敏者。⑤治疗部位皮肤有感染或皮肤病者。⑥因某种原因不能坚持或不能接受本方案者。⑦就诊前用过镇痛药、膏药、局封、针灸等其他治疗方法可能影响结果判定者。 1.4 一般资料选择病例为2009年1月～2012年10月揭西县人民医院针灸科门诊部自愿参加阿是穴温针灸加针刺后溪穴治疗棘上、棘间韧带损伤研究的患者，年龄19～68岁，纳入符合入组标准的棘上、棘间韧带损伤患者120例，按照随机单盲法分为2组。阿是穴温针灸加针刺后溪穴组60例，男32例，女28例，年龄21～68岁，病程最长8个月，最短15天；齐刺加推拿组60例，男30例，女30例，年龄19～64岁，病程最长8个月，最短13天。两组患者性别、年龄、病程等方面差异均无统计学意义(P ＞0.05)，具有可比性。 2 治疗方法 2.1 治疗组采用阿是穴温针灸加针刺后溪穴法。 阿是穴温针灸法： 取穴：阿是穴。 操作方法：先用寻按法在督脉经上找到压痛明显的地方，即为阿是穴。用华佗牌一次性无菌针灸针（规格：0.35*40 mm或0.35*50 mm，根据阿是穴处肌肉厚薄选择）直刺入阿是穴10-30 mm深（依患者胖瘦而定），使针尖到达病所，得气后即可用一次性胶布固定针体，再用2cm长的纯艾条置于针柄上施灸，在皮肤与艾条之间放置纸皮隔热，使患者耐受即可，连灸两柱后除去艾条灰烬，出针。 针刺后溪穴法： 取穴：后溪。 操作方法：在阿是穴温针灸的同时，用华佗牌一次性无菌针灸针（规格：0.30*25 mm）直刺两侧后溪穴约15 mm深，间断用捻转平补平泻法行针3次，留针30分钟后出针。 2.2 对照组采用齐刺加推拿法。 齐刺法： 取穴：阿是穴和阿是穴水平旁开0.3～0.5寸的痛点边缘处。 操作方法：用华佗牌一次性无菌针灸针（规格：0.35*40 mm）直刺入阿是穴和阿是穴水平旁开0.3～0.5寸的痛点边缘处，皆直刺入10-30 mm深（依患者胖瘦而定），以得气为度; 间断用捻转平补平泻法行针3次，留针30分钟后出针。 推拿法：起针后，医者用拇指、示指指腹沿阿是穴及两旁按揉3～5次，理顺棘上棘间韧带，并在压痛处紧压3次结束。 两组均是每天治疗1次，5次为1疗程。治疗1疗程后观察疗效。 3 统计学方法 应用SPSS17.0统计软件进行数据处理，计数资料以率(%)表示，采用x2检验。 4 疗效标准与治疗结果 4.1 疗效标准根据国家中医药管理局1994年《中医病证诊断疗效标准》[2]拟定：①治愈:腰痛消失,脊柱活动功能恢复正常; ②好转:
